摘要
抑郁症是以情绪低落、兴趣减退为核心症状的一类情感障碍类疾病,此病发病机制复杂、症状广泛、病情迁延难愈,具有高致残率、高病死率特点,严重危害个人及社会安全。目前西医治疗尚无特效药,以对症治疗为主,而研究发现中医药治疗此病具有显著优势。柴胡作为解表药,具有疏肝解郁、退热、补阳之功效,对抑郁症患者具有明确疗效。目前研究发现柴胡活性成分如柴胡皂苷、山柰酚、异鼠李素、槲皮素,柴胡药对如柴胡-郁金、柴胡-白芍、柴胡-川芎、柴胡-黄芩,柴胡配伍方剂如逍遥散、柴胡疏肝散、小柴胡汤、四逆散、柴胡加龙骨牡蛎散等抗抑郁效果显著,其机制可能与调控下丘脑-垂体-肾上腺(HPA)轴、调节微生物-肠-脑轴、调节神经递质、抑制免疫炎性反应、调节神经细胞营养因子等有关。故该文对柴胡单体、药对、配伍方剂、中成药抗抑郁的具体机制及进展进行总结,以便为后续临床应用及实验研究提供参考。
Depression is a type of affective disorder with depressed mood and diminished interest as the core symptoms,which has a complex pathogenesis,a wide range of symptoms,a prolonged illness and a high rate of disability and mortality,seriously endangering personal and social safety.There is no specific treatment available in Western medicine,but symptomatic treatment is the mainstay,and research has found that Chinese medicine has significant advantages in treating this disease.As an antidepressant,Chaihu(Bupleuri Radix)has the ability to detoxify the liver,relieve depression,reduce fever and tonify Yang,and has clear efficacy in patients with depression.Current studies have found that the active ingredients of Chaihu(Bupleuri Radix),such as saikosaponin,kaempferol,isorhamnetin and quercetin,the pairs of Chaihu(Bupleuri Radix)medicines,such as Chaihu(Bupleuri Radix)-Yujin(Curcumae Radix),Chaihu(Bupleuri Radix)-Baishao(Paeoniae Radix Alba),Chaihu(Bupleuri Radix)-Chuanxiong(Chuanxiong Rhizoma),Chaihu(Bupleuri Radix)-Huangqin(Scutellariae Radix),Chaihu(Bupleuri Radix)compatibility formula such as Xiaoyao Powder(逍遥散),Chaihu Shugan Powder(柴胡疏肝散),Xiaochaihu Decoction(小柴胡汤),Sini Powder(四逆散),Chaihu Jia Longgu Muli Powder(柴胡加龙骨牡蛎散)and etc.,have significant antidepressant effects,the mechanisms of which may be related to the regulation of the hypothalamic-pituitary-adrenal(HPA)axis,the regulation of the microbial-intestinalbrain axis,regulation of neurotransmitters,inhibition of immune inflammatory responses,and regulation of neurotrophic factors.Therefore,this paper summarizes the specific mechanisms and progress of the antidepressant effects of Chaihu(Bupleuri Radix)alone,in pairs,in combination with prescriptions and in proprietary Chinese medicines,in order to provide reference for subsequent clinical applications and experiments.
