摘要
多囊卵巢综合征(polycystic ovary syndrome,PCOS)是育龄期妇女常见的疾病之一,约60%的PCOS患者存在不同程度的胰岛素抵抗(IR),而肥胖和超重的PCOS女性则更容易出现葡萄糖代谢紊乱并引起慢性炎症。中枢神经系统被认为在PCOS的发病机制中起重要作用,下丘脑炎症是PCOS主要的中枢神经特征改变。该文主要从PCOS-IR与肥胖诱发的下丘脑炎症通路相关性进行综述,聚焦于PCOS-IR与下丘脑炎症通路相关性的研究,大致总结了PCOS-IR伴发的下丘脑炎症的主要相关通路,通过查阅大量的文献,PCOS-IR与下丘脑炎症相互影响的因素多数指向高脂饮食诱导的内分泌紊乱,进而加重下丘脑的氧化应激及炎症反应,笔者认为对于该病的研究应给予中枢神经系统足够重视而不仅局限于生殖系统,谨荐对PCOS-IR伴下丘脑炎症中下丘脑部位相关通路研究有所侧重,旨在为今后的前瞻性治疗及研究提供借鉴。
Polycystic ovary syndrome(PCOS)is one of the common diseases in women of reproductive age.About 60%of PCOS patients have varying degrees of insulin resistance(IR),while obese and overweight PCOS women are more likely to have glucose metabolism disorders and cause chronic inflammation.The central nervous system is considered to play an important role in the pathogenesis of PCOS,and hypothalamic inflammation is the main central nervous system characteristic change in PCOS.This article mainly reviews the correlation between PCOS-IR and obesity-induced hypothalamic inflammatory pathways,focusing on the correlation between PCOS-IR and hypothalamic inflammatory pathways,and roughly summarizes the main related pathways of hypothalamic inflammation in this comorbidity.Through a large number of literature review,Most of the factors for the comorbidity of PCOSIR and hypothalamic inflammation point to endocrine disorders induced by high-fat diet,which further aggravate the oxidative stress and inflammatory response of the hypothalamus.The authors believe that enough attention should be paid to the central nervous system rather than limited to the reproductive system for the study of this disease.The aim of this study is to provide reference for future prospective treatment and research.
