摘要
目的探究超声造影微血管密度成像(MVI)与乳腺癌患者血管形态、微血管密度(MVD)的关系。方法选取2020年4月至2021年10月南昌市第三医院收治的78例乳腺癌患者作为乳腺恶性肿瘤组,另选取2021年1—10月收治的50例乳腺良性肿瘤患者作为良性对照组。所有患者均进行超声造影MVI检查,术后标本行抗CD34免疫组化染色。统计并比较两组超声造影结果出现不均匀性增强、充盈缺损、血管网紊乱及血管扩张的比例;检测并比较两组MVD结果、峰值强度、增强幅度及开始至90s平均强度、造影开始增强时间;分析乳腺癌组织的MVD与MVI参数的相关性。结果乳腺恶性肿瘤组MVI造影不均匀性增强、充盈缺损、血管网紊乱及血管扩张占比均高于良性对照组,差异有统计学意义(P<0.05)。乳腺恶性肿瘤组MVD、峰值强度、增强幅度及开始至90s平均强度均高于良性对照组,差异有统计学意义(P<0.05);两组造影开始增强时间、峰值时间比较差异无统计学意义。乳腺癌组织MVD与超声造影MVI的峰值强度、增强幅度及开始至90s平均强度呈正相关(r>0,P<0.05)。结论乳腺癌患者MVI与肿瘤血管形态、MVD指标呈正相关,对评估患者肿瘤发生恶变与转移有一定提示作用。
Objective To explore the relationship between contrast-enhanced ultrasound microvessel density imaging(MVI)and vessel morphology,microvessel density(MVD)in patients with breast cancer.Methods A total of 78 patients with breast cancer were admitted to Nanchang Third Hospital from April 2020 to October 2021 were selected as the breast malignant tumor group,and another 50 patients with benign breast tumor admitted from January to October 2021 were selected as the benign control group.All underwent contrast-enhanced ultrasound MVI examination.The proportion of uneven enhancement,filling defects,vascular network disorder and vasodilation in both groups were counted and compared.MVD results,peak intensity,enhancement amplitude,average intensity from beginning to 90 s,start time of contrast enhancement and peak time in both groups were detected and compared.The correlation between MVD and MVI parameters in breast cancer tissue was analyzed.Results The proportion of uneven enhancement,filling defects,vascular network disorder and vasodilation in the breast malignant tumor group were higher than those in benign control group,and the differences were statistically significant(P<0.05).The MVD,peak intensity,enhancement amplitude and average intensity of starting to 90 s in the breast malignant tumor group were higher than those in the benign control group,the differences were statistically significant(P<0.05).There were no significant differences in the beginning enhancement time and peak time between the two groups.The MVD of breast cancer tissue was positively correlated with the peak intensity,enhancement amplitude and average intensity of starting to 90 s of microvascular density imaging(r>0,P<0.05).Conclusion MVI results are positively correlated with vessel morphology and MVD indexes in patients with breast cancer,which are of certain suggesting roles in malignant transformation and metastasis of tumors.
