三血管切面联合肺动脉血流速度对胎儿先天性心脏病的预测价值 被引量:2

Predictive value of three-vessel-trachea view combined with pulmonary artery flow velocity for fetal congenital heart diseases

摘要 目的 分析三血管切面联合肺动脉血流速度对胎儿先天性心脏病的预测价值。方法 选取2019年1月—2020年12月金华市中心医院的疑似胎儿先天性心脏病的107例孕妇作为研究对象,以胎儿超声心动图作为“金标准”,最终确认先天性心脏病胎儿79例为观察组,剩余28例胎儿为对照组。所有孕妇均采用多普勒超声诊断仪对三血管切面和肺动脉血流速度进行检测,分析三血管切面、肺动脉血流速度及两项联合检出胎儿先天性心脏病与“金标准”检出结果的一致性,分析三血管切面、肺动脉血流速度及两项联合对胎儿先天性心脏病的预测价值。结果 对照组血流加速时间(ACT)、射血时间(ET)、肺动脉最大血流速度(PSV)、减速时间(DT)、流速时间积分(VTI)分别为(44.52±8.47)ms、(211.29±25.53)ms、(122.53±13.12)cm/s、(168.64±15.21)ms、(9.67±2.25)cm、观察组ACT、ET、PSV、DT、VTI分别为(36.73±8.42)ms、(186.27±17.04)ms、(75.26±9.57)cm/s、(138.72±13.64)ms、(13.78±3.81)cm、与对照组相比,观察组ACT、ET、PSV、DT水平均降低,VTI水平升高,差异均有统计学意义(均P<0.05)。与“金标准”诊断结果相比,三血管切面和肺动脉血流参数及两项联合检出的胎儿先天性心脏病分别为62例、65例、73例。与三血管切面(78.48%)和肺动脉血流参数(82.28%)相比,两项联合对胎儿先天性心脏病的检出率(92.41%)最高,差异有统计学意义(χ^(2)=6.213,P<0.05)。与三血管切面、肺动脉血流参数单独检测相比,两项联合对胎儿先天性心脏病的预测价值较高,差异有统计学意义(P<0.05)。结论 三血管切面、肺动脉血流参数对胎儿先天性心脏病均有一定的预测价值,但两项联合对胎儿先天性心脏病的预测价值更高,为临床评估胎儿先天性心脏病提供了重要的参考依据。 Objective To analyze the predictive value of three-vessel-trachea view combined with pulmonary artery flow velocity for fetal congenital heart diseases.Methods From January 2019 to December 2020,107 pregnant women with suspected fetal congenital heart diseases were selected as study object,fetal echocardiography was taken as the"gold standard",79 fetuses diagnosed as congenital heart diseases definitely were selected as observation group,and the other 27 fetuses were selected as control group.Doppler ultrasound diagnostic instrument was used to detect three-vessel-trachea view and pulmonary artery flow velocity,the consistency of three-vessel-trachea view,pulmonary artery flow velocity,the combination of the two methods was analyzed,the predictive value of three-vessel-trachea view,pulmonary artery flow velocity,the combination of the two methods for fetal congenital heart diseases was analyzed.ResultsAcceleration time(ACT),ejection time(ET),peak systolic velocity(PSV),deceleration time(DT),and velocity time integral(VTI)in control group were(44.52±8.47)ms,(211.29±25.53)ms,(122.53±13.12)cm/s.(168.64±15.21)ms,and(9.67±2.25)cm,respectively;ACT,ET,PSV,DT,and VTI in observation group were(36.73±8.42)ms,(186.27±17.04)ms,(75.26±9.57)cm/s,(138.72±13.64)ms,and(13.78±3.81)cm,respectively.Compared with control group.ACT,ET,PSV,and DT in observation group decreased significantly,VTI increased significantly,there were statistically significant differences(P<0.05).Compared with"gold standard",the numbers of fetuses with congenital heart diseases detected by three-vessel-trachea view,pulmonary artery flow velocity,the combination of the two methods were 62,65,and 73.respectively.Compared with three-vessel-trachea view(78.48%),pulmonary artery flow velocity(82.28%),the detectoin rate of congenital heart diseases detected by the combination of the two methods(92.41%)was the highest,there was statistically significant diference(X°=6.213,P<0.05).Compared with single detection of three-vessel-trachea view and pulmonary artery flow velocity,the predictive value of the combination of the two methods was higher,there was statistically significant difference(P<O.05).Conclusion Three-vessel-trachea view and pulmonary artery flow velocity have a certain predictive value for fetal congenital heart diseases,but the combination of the two methods has a higher predictive value for fetal congenital heart diseases,which provides an important reference for clinical evaluation of fetal congenital heart diseases.
