摘要
目的探讨儿童支气管哮喘合并过敏性鼻炎患者与外周血单核细胞-淋巴细胞比值(MLR)、中性粒细胞-淋巴细胞比值(NLR)及血液常规指标之间的关系。方法选取2020年7月至2021年7月南京医科大学附属无锡人民医院收治哮喘患儿120例,其中60例为哮喘合并过敏性鼻炎患儿,60例为单纯哮喘患儿,选择同期60例健康体检儿童作为健康对照组,采用Kruskal-Wallis检验对3组数据进行比较,Spearman相关系数分析MLR、NLR和嗜酸性粒细胞之间的线性关系,评估受试者工作曲线(ROC)下的面积,以确定MLR、NLR预测儿童哮喘合并过敏性鼻炎的灵敏度和特异性。结果哮喘合并过敏性鼻炎组的MLR、NLR和嗜酸性粒细胞显著高于健康对照组(P<0.05),MLR和NLR显著高于单纯哮喘组(P<0.05),单纯哮喘组与哮喘合并过敏性鼻炎组嗜酸性粒细胞差异无统计学意义(P>0.05)。哮喘合并过敏性鼻炎组的MLR与嗜酸性粒细胞之间呈正相关关系。MLR、NLR对哮喘合并过敏性鼻炎患儿的预测临界值分别是0.162、1.038,受试者工作曲线下面积分别为0.702、0.648。结论MLR和NLR可作为评估儿童哮喘合并过敏性鼻炎炎症反应的早期初步诊断指标。
Objective To investigate the relationship between peripheral blood monocyte-to-lymphocyte ratio(MLR),neutrophil-to-lymphocyte ratio(NLR)and blood routine indexes in children with asthma and allergic rhinitis.Methods A total of 120 children with asthma admitted to Wuxi People’s Hospital Affiliated to Nanjing Medical University from July,2020 to July,2021 were selected for the study,including 60 children with asthma and allergic rhinitis and 60 children with simple asthma.60 healthy children who underwent physical examination during the same period were selected as the healthy control group.Kruskal-Wallis test was used to compare the data among these three groups.Spearman correlation coefficient was used to analyze the linear relationship between MLR,NLR and eosinophils.The area under the receiver operating curve(AUROC)was calculated to determine the sensitivity and specificity of MLR and NLR in predicting asthma and allergic rhinitis in children.Results MLR,NLR and eosinophils of the asthma and allergic rhinitis group were significantly higher than those of the healthy control group(P<0.05),and there was no significant difference in eosinophils between the simple asthma group and the asthma and allergic rhinitis group(P>0.05),while MLR and NLR of the asthma and allergic rhinitis group were significantly higher than the simple asthma group(P<0.05).In the asthma and allergic rhinitis group,MLR was positively correlated with eosinophils.MLR,NLR,eosinophils of children with asthma and allergic rhinitis had the critical predicting value of 0.162 and 1.038,while the area under the receiver-operating curve were 0.702 and 0.648,respectively.Conclusion MLR and NLR can be used as preliminary diagnostic indicators to evaluate the inflammatory response of children with asthma and allergic rhinitis.
