摘要
目的研究在进行早产剖宫产手术时保留胎膜囊剖宫产术式的临床效果。方法选取2021年6月—2023年10月贵港市妇幼保健院产科收治的65例需行剖宫产术的未破膜早产患者为研究对象,有31例采取保留胎膜囊剖宫产术,其中成功者25例作为研究组,34例采取传统子宫下段剖宫产术的患者为对照组,分析保留胎膜囊剖宫产术组实施效果情况,比较两组的手术开始到胎儿娩出时间、羊水量、术中出血、早产儿出生体质量、出生后1、5 min的Apgar评分情况。结果实施保留胎膜囊剖宫产术成功率为80.65%。两组患者手术开始到胎儿娩出时间、羊水量、术中出血量比较,差异无统计学意义(P均>0.05)。两组新生儿体质量比较,差异无统计学意义(P>0.05)。新生儿出生后1、5 min,研究组Apgar评分分别为(9.92±0.28)分、(10±0)分均优于对照组的(9.74±0.67)分、(9.91±0.05)分,差异有统计学意义(t=7.245、8.979,P均<0.05)。结论在早产剖宫产术时实施保留胎膜囊剖宫产术,能够提高早产儿出生评分,降低早产儿窒息率。
Objective To study the clinical effect of cesarean section with preserved membrane sac during preterm de⁃livery.Methods A total of 65 patients with unbroken membrane preterm delivery requiring cesarean section admitted to the Obstetrics Department of Guigang Maternal and Child Health Hospital from June 2021 to October 2023 were se⁃lected as the study objects.31 patients underwent cesarean section with preserved fetal membrane capsule,of which 25 were successful as the study group and 34 patients underwent traditional lower uterine cesarean section as the con⁃trol group.The results of cesarean section with preserved membrane capsule were analyzed,and the time from opera⁃tion to delivery,amniotic fluid volume,intraoperative bleeding,birth weight of preterm infants,and Apgar score 1 and 5 minutes after birth were compared between the two groups.Results The success rate of cesarean section with pre⁃served membranes was 80.65%.There was no statistically significant difference in the time from operation to fetal de⁃livery,amniotic fluid volume and intraoperative blood loss between the two groups(all P>0.05).There was no statisti⁃cally significant difference in body mass between the two groups(P>0.05).At 1 min and 5 min after birth,the Apgar score of the study group were(9.92±0.28)points and(10±0)points,respectively,which were better than those of the control group(9.74±0.67)points and(9.91±0.05)points,and the differences were statistically significant(t=7.245,8.979,both P<0.05).Conclusion Cesarean section with preserved membranes can improve the birth score of preterm infants and reduce the asphyxia rate of preterm infants.
