飞秒激光辅助白内障超声乳化联合区域折射多焦人工晶体植入术治疗高度近视合并白内障的临床效果

Clinical effects of femtosecond laser assisted phacoemulsification combined with regional refractive multifocal intraocular lens implantation in patients with high myopia and cataract

摘要 目的观察飞秒激光辅助白内障超声乳化联合区域折射多焦人工晶体(LS-313MF15,荷兰)植入术治疗高度近视合并白内障的临床效果及视觉质量。方法回顾性分析我院2019-3/2021-3期间手术的高度近视合并白内障患者共67例90眼,分为三组:A组:飞秒激光辅助联合LS-313MF15植入组共20例33眼;B组:普通白内障超声乳化联合LS-313MF15植入组共24例28眼;C组:飞秒激光辅助联合单焦点人工晶体(Proming ALD)植入组共23例29眼。术后随访3个月,收集三组患者视力恢复情况,脱镜率,夜间炫光、光晕发生率,总高阶像差,绘制离焦曲线。结果术后3个月,裸眼远视力:A组、B组明显优于C组(P<0.05),A、B两组间无明显差异(P>0.05);矫正远视力三组间无显著差异(P>0.05),裸眼中、近视力:A组优于B组,B组优于C组。脱镜率:A组:96.9%,B组:92.8%,C组:20.6%,A、B组明显高于C组(χ^(2)=53.076,P<0.05)。夜间炫光、光晕发生率:A组:6.0%,B组:10.7%,C组:3.4%,三组间无显著差异(χ^(2)=1.239,P>0.05)。A、C组患者总高阶像差均显著低于术前且低于B组(P<0.05)。与B、C两组相比,A组患者在不同离焦范围内曲线平坦、视力较好。结论对于高度近视合并白内障患者,飞秒激光辅助下同时植入区域折射多焦人工晶体能获得良好的全程视力,较高的术后视觉质量。 Objective To observe the clinical effection and visual quality of femtosecond laser assisted cataract phacoemulsification(FLACS)combined with regional refractive multifocal intraocular lens(LS-313MF15,Holland)implantation in patients with high myopia and cataract.Method A total of 67 patients(90 eyes)with high myopia and cataract who underwent surgery in our hospital from March 2019 to March 2021 were retrospectively analyzed.They were divided into three groups,group A(20 patients 33 eyes):FLACS assisted LS-313MF15 implantation,group B(24 patients 28 eyes):ordinary cataract phacoemulsification combined with LS-313MF15 implantation,group C(23 patients 29 eyes):FLACS assisted mono-focal IOL(Proming,ALD)implantation.The patients were followed up for 3 months,the visual acuity recovery,spectacle independence,incidences of glare and halo at night,total higher-order aberration(HOA),defocus curve of the three groups were collected.Result At 3 months after surgery:the uncorrected distance visual acuity of group A and group B were significantly better than group C(<0.05),there was no significant difference between group A and group B(>0.05);There was no significant difference in corrected distance visual acuity among three groups(>0.05).Uncorrected intermediate and near visual acuity:group A was better than group B,group B was better than group C.The rate of spectacle independence:group A:96.9%,group B:92.8%,group 2C:20.6%,group A and group B were significantly higher than group C(χ^(2)=53.076,<0.05).Incidences of glare and halo at night:group A:6.0%,group B:10.7%,group C:3.4%,there was no significant difference among three groups(χ^(2)=1.239,>0.05).The HOA in group A and group C were significantly lower than those before surgery and lower than group B(<0.05).Defocus curve:compared to group B and group C,the curve of group A was flat and the visual acuity was good in different defocus ranges.Conclusion For the patients with high myopia combined with cataract,simultaneous implantation of LS-313MF15 assisted by FLACS could obtain better visual acuity,higher postoperative visual quality.
