摘要
目的探讨磁共振成像(MRI)在卵巢肿瘤诊断中的临床价值。方法选取62例卵巢肿瘤患者,均行MRI检查,以病理检查结果为卵巢肿瘤检查的金标准,评估MRI检查对卵巢肿瘤良恶性诊断的价值,并分析MRI影像学表现、MRI对卵巢肿瘤的定性诊断准确度,另对恶性卵巢肿瘤进行MRI分期,比较病理分期结果。结果病理检查卵巢肿瘤患者良性35例,恶性27例;经MRI检查诊断卵巢肿瘤的准确度95.16%、敏感度92.86%、特异度97.06%,与病理检查结果一致性较好(Kappa=0.90);在定性诊断上,MRI对单纯囊肿、巧克力囊肿、浆膜性囊腺癌、成熟性畸胎瘤、乳头状囊腺癌、黏液性囊腺癌、未成熟畸胎瘤、颗粒细胞癌诊断准确度分别为92.31%、88.89%、100%、100%、91.67%、100%、100%、100%;以手术病理检查结果为金标准,MRI对Ⅰ、Ⅱ、Ⅲ期诊断符合率分别为100%、92.86%、87.50%,术前分期准确度为92.59%,与金标准一致性较好(Kappa=0.88)。结论卵巢肿瘤采用MRI检查能准确鉴别良恶性,定性诊断以及术前分期准确度高,值得临床推广与应用。
Objective To investigate the clinical value of magnetic resonance imaging(MRI)in the diagnosis of ovarian tumors.Methods Sixty-two patients with ovarian tumors were examined by MRI,and the results of pathological examination were used as the gold standard of ovarian tumor examination.The value of MRI examination in the diagnosis of benign and malignant ovarian tumors was evaluated,and the imaging manifestations of MRI and the accuracy of MRI in the qualitative diagnosis of ovarian tumors were analyzed.Results There were 35 benign and 27 malignant ovarian tumors.The accuracy,sensitivity and specificity of MRI in the diagnosis of ovarian tumors were 95.16%,92.86%and 97.06%,respectively,which were consistent with the results of pathological examination(Kappa=0.90).In qualitative diagnosis,MRI of simple cyst,chocolate cyst,serous cystadenocarcinoma,maturity teratoma,papillary cystadenocarcinoma,mucous cystadenocarcinoma,immature teratoma,granular cell carcinoma diagnosis accuracy were 92.31%,88.89%,100%,100%,91.67%,100%,100%,100%;With surgical pathologic examination results as the gold standard,magnetic resonance imaging(MRI)for phase Ⅰ,Ⅱ,Ⅲdiagnosis coincidence rate is 100%,92.86%and 87.50%respectively,preoperative staging accuracy was 92.59%,and the gold standard consistency(Kappa=0.88).Conclusion MRI can accurately distinguish benign and malignant ovarian tumors,and the accuracy of qualitative diagnosis and preoperative staging is high,which is worthy of clinical promotion and application.
