激光光凝结合抗VEGF药物在治疗糖尿病黄斑水肿中的实际效果研究

Study on the Practical Effectiveness of Laser Photocoagulation Combined with Anti-VEGF Drugs in the Treatment of Diabetic Macular Edema

摘要 目的 研讨激光光凝结合抗血管内皮生长因子(vascular endothelial growth factor, VEGF)药物在治疗糖尿病黄斑水肿(diabetic macular edema, DME)中的实际效果。方法 选取2020年6月—2023年1月济南市济阳区人民医院收治的DME患者123例为研究对象,按照随机数表法分对照组60例与观察组63例,分别予以单纯激光光凝治疗以及激光光凝结合抗VEGF药物治疗,比较两组的临床疗效、不同治疗时间的临床相关指标[最佳矫正视力(best corrected vision, BCVA)、黄斑中心厚度(macular center thickness, MCT)及视网膜新生血管面积(retinal neovascularization area, RNA)]以及并发症发生情况。结果 观察组治疗总有效率(96.83%)明显高于对照组(86.67%),差异有统计学意义(P<0.05)。治疗后,观察组BCVA、MCT以及RNA均低于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论 激光光凝结合抗VEGF药物方案用于DME患者的治疗,能够明显提升临床疗效,缩小黄斑中心厚度及视网膜新生血管面积,加快患者视力恢复,同时保障治疗的安全性。 Objective To examine the study of the practical effect of laser photocoagulation combined with anti-vascular endothelial growth factor(VEGF)drugs in the treatment of diabetic macular edema(DME).Methods 123 cases of DME patients admitted to Jinan Jiyang District People's Hospital from June 2020 to January 2023 were selected as study subjects,and were divided into 60 cases in the control group and 63 cases in the observation group according to the random number table method,and were treated with laser photocoagulation alone and laser photocoagulation combined with anti-VEGF drugs,respectively.The clinical efficacy,the clinical indicators[best corrected vision(BCVA),macular center thickness(MCT)and retinal neovascularization area(RNA)]at different treatment times and the occurrence of complications of the two groups was compared.Results The total effective rate of treatment in the observation group(96.83%)was significantly higher than that in the control group(86.67%),and the difference was statistically significant(P<0.05).After treatment,BCVA,MCT,and RNA of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the complication rate between the two groups(P>0.05).Conclusion Laser photocoagulation combined with anti-VEGF drug regimen for the treatment of DME patients can significantly improve the clinical efficacy,reduce the thickness of the macular center and the area of retinal neovascularization,and accelerate the recovery of patients'visual acuity,while guaranteeing the safety of the treatment.
