摘要
目的 分析西格列汀与胰岛素泵强化方案联合治疗初诊2型糖尿病的效果。方法 选取2022年6月—2023年5月南平市第一医院内分泌科收治的290例初诊2型糖尿病患者,按随机数表法分为研究组与对照组,各145例。对照组采取胰岛素泵治疗,研究组采取西格列汀与胰岛素泵强化方案联合治疗。针对两组血糖达标时间、胰岛素每日使用量、胰岛素抵抗指数(homeostatic model assessment of insulin resistance,HOMA-IR)、胰岛β细胞功能指数(homeostasis model assessment-β, HOMA-β),以及不良反应发生率进行比较。结果 研究组血糖达标时间短于对照组,差异有统计学意义(P<0.05),胰岛素每日使用量较对照组低,差异有统计学意义(P<0.05)。治疗8周后,研究组HOMA-IR指标较对照组低,差异有统计学意义(P<0.05);HOMA-β指标较对照组高,差异有统计学意义(P<0.05)。两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论 西格列汀与胰岛素泵强化方案联合治疗能够促进初诊2型糖尿病患者血糖达标,减少胰岛素用量,改善其胰岛功能,安全性较佳,适于临床应用。
Objective To analyze the effect of sitagliptin and insulin pump in the treatment of newly diagnosed type 2 diabetes.Methods A total of 290 patients with newly diagnosed type 2 diabetes admitted to the Department of Endocrinology of the First Hospital of Nanping from June 2022 to May 2023 were selected and divided into the study group and the control group according to the random number table method,with 145 cases in each group.The control group was treated with insulin pump therapy,and the study group was treated with sitagliptin combined with insulin pump intensive therapy.The time of blood glucose reaching target,daily insulin dosage,homeostatic model assessment of insulin resistance(HOMA-IR)and homeostasis model assessment-β(HOMA-β)were compared between the two groups.And the incidence of adverse reactions were compared.Results The time of blood glucose reaching the standard in the study group was shorter than that in the control group,and the difference was statistically significant(P<0.05).The daily dosage of insulin in the study group was lower than that in the control group,and the difference was statistically significant(P<0.05).After 8 weeks of treatment,the HOMA-IR index of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).The HOMA-βindex was higher than that of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination therapy of sitagliptin and insulin pump can promote the blood glucose standard of newly diagnosed type 2 diabetes patients,reduce insulin dosage,improve their islet function,and have good safety,which is suitable for clinical application.
