摘要
目的探讨人工股骨头置换术治疗高龄老年人股骨颈骨折的临床效果研究。方法将2022年1—12月在本院骨科手术治疗的64例高龄老年股骨颈骨折患者随机分为两组,对照组使用全髋关节置换术,观察组使用人工股骨头置换术,对比两组的各项手术指标、并发症、恢复效果、髋关节功能指标。结果观察组手术时间、术中出血量、术后引流量、术后首次下床活动时间、住院时间均少于对照组(P<0.05);观察组假体脱位、假体无菌性松动、关节内感染、下肢深静脉血栓等并发症发生率低于对照组(P<0.05);观察组髋关节功能优良率与对照组相当(P>0.05);观察组术后3个月、6个月的Harris评分均与对照组相当(P>0.05)。结论人工股骨头置换术治疗高龄老年人股骨颈骨折的临床效果显著,创伤较小,出血量少,术后恢复速度快,并发症发生率低,髋关节功能恢复好。
Objective To investigate clinical effect of artificial femoral head replacement surgery for femoral neck fractures of elderly patients.Methods The paper chose 64 elderly patients with femoral neck fractures with orthopedic surgery in our hospital from January 2022 to December 2022,and divided them into two groups randomly.Control group was treated with total hip arthroplasty and observation group with artificial femoral head arthroplasty.Surgical indicators,complications,recovery effect,and hip joint function indicators between two groups were compared.Results Observation group had shorter surgical time,intraoperative bleeding volume,postoperative drainage volume,first postoperative ambulation time,and hospital stay than control group(P<0.05).Incidence of complications such as prosthetic dislocation,aseptic loosening,intra articular infection,and deep vein thrombosis in observation group was lower than control group(P<0.05).Excellent and good rate of hip joint function in observation group was comparable to control group(P>0.05).Harris scores of observation group at 3 and 6 months after surgery were comparable to control group(P>0.05).Conclusion Artificial femoral head replacement can achieve significant clinical effect for femoral neck fractures of the elderly,with minimal trauma,minimal bleeding,fast postoperative recovery,low incidence of complications,and good recovery of hip joint function.
