超声造影联合剪切波弹性成像在预测甲状腺乳头状癌颈部淋巴结转移中的价值

Value of Contrast-enhanced Ultrasound Combined with Shear-wave Elastography in Predicting Cervical Lymph Node Metastasis of Papillary Thyroid Carcinoma

摘要 目的 探讨超声造影(CEUS)定量参数联合剪切波弹性值在甲状腺乳头状癌颈部淋巴结转移的诊断价值。方法 回顾性分析本院甲状腺乳头状癌患者63例共计102个增大淋巴结,按病理结果分为转移组(58例)和未转移组(44例)。分别比较两组淋巴结的常规超声、CEUS特征及剪切波弹性值,并绘制受试者工作特征曲线(ROC)分析超声造影联合剪切波弹性成像在甲状腺乳头状癌颈部转移淋巴结中的诊断效能。结果 转移组最大弹性值(E_(max))和平均弹性值(E_(mean))均高于未转移组,差异均有统计学意义(P<0.05)。转移组淋巴结以混合性灌注,不均匀增强为主(薄环42例,厚环16例),且更易出现灌注缺损;未转移组淋巴结以向心性灌注,均匀增强为主(薄环14例,厚环30例)。转移组淋巴结始增时间、达峰时间、峰值强度及曲线下面积均小于未转移组,差异均有统计学意义(P<0.05)。ROC曲线分析表明超声造影联合剪切波弹性成像诊断甲状腺乳头状癌颈部淋巴结转移的曲线下面积0.92,敏感性为92%,特异性为93%,均高于单一方法应用。结论 超声造影联合剪切波弹性成像在诊断甲状腺乳头状癌颈部淋巴结转移中具有较好的临床应用价值。 Objective To evaluate the diagnostic value of quantitative parameters of contrast-enhanced ultrasound(CEUS)combined with shear wave elasticity in cervical lymph node metastasis of papillary thyroid carcinoma.Methods A total of 102 enlarged lymph nodes in 63 patients with papillary thyroid carcinoma in our hospital were retrospectively analyzed.According to pathological results,they were divided into metastatic group(58 cases)and non-metastatic group(44 cases).The conventional ultrasonography,CEUS features and shear wave elastic values of lymph nodes in the two groups were compared respectively.Receiver operating characteristic curve(ROC)was plotted to analyze the diagnostic efficiency of contrast-enhanced ultrasound combined with shear-wave elastography in cervical metastatic lymph nodes of papillary thyroid carcinoma.Results The maximum elastic value(E_(max))and mean elastic value(E_(mean))in the metastatic group were higher than those in the non-metastatic group,with statistical significance(P<0.05).The lymph nodes in the metastatic group were mainly mixed perfusion with uneven enhancement(thin ring 42 cases,thick ring 16 cases),and perfusion defect was more likely.Lymph nodes in the non-metastatic group were mainly perfused centrially and enhanced uniformly(14 cases with thin ring and 30 cases with thick ring).Lymph nodes in the metastatic group had a smaller initial growth time,peak time,peak intensity and area under the curve than those in the non-metastatic group.The difference was statistically significant(P<0.05).ROC curve analysis showed that the area under the curve of CEUS combined with shear wave elastography in the diagnosis of cervical lymph node metastasis of papillary thyroid cancer was 0.92,the sensitivity was 92%,and the specificity was 93%,which were higher than that of single method.Conclusion Contrast-enhanced ultrasound combined with shear-wave elastography has good clinical value in the diagnosis of cervical lymph node metastasis of papillary thyroid carcinoma.
