摘要
目的:研究标准通道经皮肾镜取石术(SPCNL)联合输尿管软镜钬激光碎石术(FURS)对上尿路结石(UUTC)患者的影响。方法:选取2020年5月—2022年5月大埔县人民医院收治的73例UUTC患者作为研究对象。将就诊序号编号,根据随机数表法将其分为观察组(n=38)和对照组(n=35)。对照组给予SPCNL,观察组给予SPCNL联合FURS。比较两组围手术期指标及结石清除率,术前、术后7 d排尿情况、肾功能指标及并发症。结果:观察组手术时间长于对照组,术中出血量少于对照组,住院时间短于对照组,结石清除率高于对照组,差异有统计学意义(P<0.05)。术后7 d,两组最大尿流速率(Qmax)、最大尿道压力(MUP)及单次排尿量(UV)较术前升高,观察组Qmax、MUP及UV均高于对照组,差异有统计学意义(P<0.05)。术后7 d,两组血清肌酐(Scr)及血尿酸(UA)及β_(2)-微球蛋白(β_(2)-MG)水平较术前降低,观察组UA、Scr及β_(2)-MG水平均低于对照组,差异有统计学意义(P<0.05)。两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论:SPCNL联合FURS可有效提高UUTC患者的结石清除率,恢复术后排尿情况及肾功能,且不增加并发症。
Objective:To study the effect of standard channel percutaneous nephrolithotomy(SPCNL)combined with flexible ureteroscopy holmium laser lithotripsy(FURS)on patients with upper urinary tract stones(UUTC).Method:A total of 73 UUTC patients treated in Dabu County People's Hospital from May 2020 to May 2022 were selected as the study objects.The patients'visit number were numbered,and they were divided into observation group(n=38)and control group(n=35)according to random number table method.The control group was given SPCNL,and the observation group was given SPCNL combined with FURS.Perioperative indexes,stone clearance rate,urination condition,renal function indexes before operation and after 7 d of operation and complications were compared between the two groups.Result:The operation time of the observation group was longer than that of the control group,the intraoperative blood loss volume was less than that of the control group,the hospitalization time was shorter than that of control group,and the stone clearance rate was higher than that of the control group,the differences were statistically significant(P<0.05).After 7 d of operation,the maximum urine flow rate(Qmax),maximum urethral pressure(MUP)and single urine volume(UV)of the two groups were higher than those before operation,and the Qmax,MUP and UV of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).After 7 d of operation,serum creatinine(Scr),serum uric acid(UA)andβ_(2)-microglobulin(β_(2)-MG)levels in the two groups were lower than those before operation,and the levels of UA,Scr andβ_(2)-MG in observation group were lower than those in control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion:SPCNL combined with FURS can effectively improve the stone clearance rate of UUTC patients,restore postoperative urination condition and renal function,and do not increase complications.
