绝经后女性血脂水平紊乱与骨代谢和骨密度的相关性

Correlation between dyslipidemia and bone metabolism and bone mineral density in postmenopausal women

摘要 目的 探讨绝经后血脂紊乱女性血脂指标与骨代谢及不同部位骨密度的关系。方法 121例符合纳入标准的女性患者,搜集其一般信息包括年龄、体质量指数(body mass index, BMI),检测其血脂指标[总胆固醇(total cholesterol, TC)、三酰甘油(triglyceride, TG)低密度脂蛋白胆固醇(low-density lipoprotein cholesterol, LDL-C)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol, HDL-C)]、骨代谢标志物[Ⅰ型胶原氨基端延长肽(type 1 procollagen amino-terminal peptide, P1NP)、β-降解胶原产物(β-cross-linked C-telopeptide of typeⅠcollagen, β-CTX)、骨钙素(osteocalcin, OC)]、不同部位骨密度(bone mineral density, BMD)。使用Spearman相关性分析、多元线性回归分析血脂指标与骨代谢标志物、不同部位BMD的相关性。结果 Spearman相关性分析显示高脂血症女性TC与P1NP(r=-0.45,P<0.001),腰椎BMD(r=-0.31,P<0.001)和股骨颈BMD(r=-0.24,P=0.007)呈负相关。LDL-C与OC(r=-0.28,P=0.002),P1NP(r=-0.21,P=0.023),腰椎BMD(r=-0.24,P=0.009)呈负相关。多元线性回归分析显示TC与腰椎BMD间呈负相关(β=-0.432,P<0.001)。结论 绝经后高脂血症女性TC与LDL-C水平升高与骨形成降低具有相关性,TC与腰椎BMD降低关联密切,本研究并未发现TG和HDL-C对各部位骨密度无明显影响。 Objective To investigate the connection between various lipid indices and bone metabolism and bone mineral density at diverse sites in postmenopausal women with dyslipidemia.Methods Information was collected on 121 female patients who fulfilled the eligibility requirements for the study,including their age,body mass index(BMI),lipid indices[total cholesterol(TC),triacylglycerol(TG),low-density lipoprotein cholesterol(LDL-C),and high-density lipoprotein cholesterol(HDL-C)],markers of bone metabolism osteocalcin,type 1 procollagen amino-terminal peptide(P1NP),andβ-cross-linked C-telopeptide of typeⅠcollagen(β-CTX),and bone mineral density(BMD)at various sites.Spearman rank correlation analysis and multiple linear regression were employed in order to investigate the connections between lipid indices,bone metabolic biomarkers,and BMD at different sites.Results In dyslipidemic women,Spearman correlation analysis revealed low inverse correlation between TC and P1NP(r=-0.45,P<0.001),low negative correlations between lumbar spine(r=-0.31,P<0.001)and femoral neck BMDs(r=-0.24,P=0.007).Negative interaction with P1NP(r=-0.21,P=0.023).The results of multiple linear regression analysis revealed a significant relation between TC and lumbar spine BMD in addition to a negative correlation between the two(β=-0.432,P<0.001).Conclusion The results of the data analysis showed a correlation between elevated TC and LDL-C levels and reduced osteogenesis in postmenopausal women with dyslipidemia,and a strong association between TC and reduced BMD in the lumbar spine,while no significant correlation of TG and HDL-C on BMD at any site was found in this study.
