摘要
【目的】观察化瘀消癥方联合地屈孕酮对宫腔镜子宫内膜息肉切除术(TCRP)后复发的防治作用,以探讨化瘀消癥方预防TCRP术后息肉复发的临床应用价值。【方法】将70例子宫内膜息肉行TCRP术后的血瘀证患者随机分为对照组和试验组,每组各35例。对照组在TCRP术后给予地屈孕酮治疗,试验组给予化瘀消癥方联合地屈孕酮治疗,2组患者均连续治疗3个月经周期。观察2组患者术前和术后3、6个月子宫内膜厚度、中医证候积分的变化情况,比较2组患者术后3、6个月的复发情况,并评价2组患者的临床疗效。【结果】(1)疗效方面,治疗3个月经周期后,试验组的总有效率为100.00%(35/35),对照组为88.57%(31/35);组间比较(秩和检验),试验组的疗效明显优于对照组,差异有统计学意义(P<0.01)。(2)复发率方面,试验组患者术后3、6个月的复发率和总复发率均明显低于对照组,差异均有统计学意义(P<0.05)。(3)子宫内膜方面,术后3、6个月,2组患者的子宫内膜厚度均较术前变薄(P<0.05),且试验组术后3、6个月的子宫内膜厚度均较对照组明显变薄,差异均有统计学意义(P<0.05)。(4)中医证候积分方面,术后3、6个月,2组患者的中医证候积分均较术前降低(P<0.05),且试验组术后3、6个月的中医证候积分均较对照组明显降低,差异均有统计学意义(P<0.05)。【结论】化瘀消癥方能促进TCRP术后患者月经恢复正常水平,降低子宫内膜厚度,并可有效预防疾病复发,临床疗效显著。
Objective To observe the preventive and curative effects of Huayu Xiaozheng Recipe(mainly with the actions of dissolving stasis and eliminating abdominal mass)combined with Dydrogesterone on the recurrence of endometrial polyps after trans-cervical resection of polyps(TCRP),and to explore the value of Huayu Xiaozheng Recipe in preventing the recurrence of endometrial polyps after TCRP in clinic.Methods Seventy patients with endometrial polyps(EP)after TCRP and with blood stasis syndrome were randomly divided into a control group and a trial group,with 35 cases in each group.The control group was treated with Dydrogesterone orally after TCRP,and the trial group was treated with Huayu Xiaozheng Recipe combined with Dydrogesterone orally after TCRP.The patients in both groups were treated for 3 consecutive menstrual cycles.The changes of endometrial thickness and traditional Chinese medicine(TCM)syndrome scores of the two groups before the operation and 3 and 6 months after the operation were observed,and the recurrence rate of the two groups 3 and 6 months after the operation were compared.After treatment,the clinical efficacy of the two groups was evaluated.Results(1)After 3 menstrual cycles of treatment,the total effective rate of the trial group was 100.00%(35/35),while that of the control group was 88.57%(31/35).The intergroup comparison(tested by rank-sum test)showed that the therapeutic effect of the trial group was significantly superior to that of the control group,and the difference was statistically significant(P<0.01).(2)Three and 6 months after the operation,the recurrence rate and the total recurrence rate in the trial group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).(3)Three and 6 months after the operation,the endometrial thickness in the two groups was thinner than that before the operation(P<0.05).The intergroup comparison showed that the endometrial thickness in the trial group 3 and 6 months after the operation was significantly thinner than that in the control group,and the difference was statistically significant(P<0.05).(4)Three and 6 months after the operation,the TCM syndrome scores of the two groups were lower than those before the operation(P<0.05).The intergroup comparison showed that the TCM syndrome scores of the trial group 3 and 6 months after the operation were significantly lower than those of the control group,and the differences were statistically significant(P<0.05).Conclusion Huayu Xiaozheng Recip can promote the recovery of postoperative menstruation of post-TCRP patients,reduce the endometrial thickness,and prevent the recurrence of endometrial polyps,with remarkable clinical efficacy.
