摘要
非酒精性脂肪性肝病(NAFLD)是全球常见的慢性肝病之一,发病机制复杂。清道夫受体B类蛋白CD36是一种重要的脂肪酸转运体,在NAFLD患者血浆及肝脏中表达升高。文章从肝脏脂质代谢、肠道脂质吸收、胰岛素抵抗、肝脏炎症反应4个方面归纳总结CD36参与脂肪酸摄取、脂质从头合成、β氧化、乳糜微粒形成、胰岛素信号及炎症信号传导在NAFLD中的触发作用及相关机制,并从中药复方、成药、有效成分方面综述中医药针对CD36发挥抗NAFLD作用研究进展,分析CD36在NAFLD疾病中的治疗潜力,为治疗策略提供新思路。
Non-alcoholic fatty liver disease(NAFLD)is one of the most common chronic liver diseases worldwide with complex pathogenesis.The scavenger receptor B protein CD36 is an important fatty acid transporter that is upregulated in the plasma and liver of NAFLD patients.The article provides an overview of the promoting effects and related mechanisms of CD36 in fatty acid uptake,de novo lipogenesis,beta-oxidation,chylomicron formation,insulin signaling,and inflammatory signaling in NAFLD,it summarizes these aspects from four perspectives:hepatic lipid metabolism,intestinal lipid absorption,insulin resistance,and hepatic inflammatory response.And it also provides a comprehensive review of the research progress in traditional Chinese medicine(TCM)targeting CD36 for its anti-NAFLD effects,including TCM formulae,patent drugs,and active compounds.The article analyzes the therapeutic potential of CD36 in NAFLD and offers new insights for treatment strategies.
