2018—2021年北京市丰台区腹泻病例弯曲菌耐药特征分析 被引量:1

Characteristics of antimicrobial resistance of Campylobacter Spp.in diarrhea cases in Fengtai District of Beijing from 2018 to 2021

摘要 目的了解北京市丰台区弯曲菌的耐药率和耐药谱,对空肠弯曲菌和结肠弯曲菌耐药率进行比较分析。方法采用双孔滤膜法对辖区哨点医院收集的2018—2021年腹泻病例粪便标本进行弯曲菌的分离培养、实时荧光PCR鉴定、全自动生化鉴定及飞行质谱鉴定,采用琼脂稀释法对弯曲菌进行药敏试验,并进行耐药分析。组间计数资料的比较应用χ^(2)检验,P<0.05为差异有统计学意义。结果共收集粪便标本1354份,弯曲菌检出率8.12%,其中空肠弯曲菌82.73%,结肠弯曲菌17.27%。弯曲菌对萘啶酸、环丙沙星和四环素的耐药率最高,均为73.64%,且2019年弯曲菌对萘啶酸、环丙沙星和四环素的耐药率高于2018年和2021年。空肠弯曲菌对红霉素、阿奇霉素、庆大霉素、链霉素、泰利霉素和克林霉素的耐药率低于结肠弯曲菌。空肠弯曲菌多重耐药率为36.26%,耐药谱以氟喹诺酮类-四环素类-氯霉素类抗生素三重耐药为主。结肠弯曲菌多重耐药率为57.89%,耐药谱以氟喹诺酮类-四环素类-氨基糖甙类-大环内酯类-林可酰胺类抗生素五重耐药为主。结论北京市丰台区腹泻病例弯曲菌的耐药情况均较为严重,应加强对弯曲菌的监测和防控。 Objective To understand the drug resistance rate and spectrum of Campylobacter spp.in Fengtai District,and to compare and analyze the drug resistance rates of Campylobacter jejuni(C.jejuni)and Campylobacter coli(C.coli).Methods The stool samples of diarrhea cases in sentinel hospitals from 2018 to 2021 were collected by double-hole filtration membrane method for Campylobacter isolation and culture,real-time PCR(polymerase chain reaction)identification,automatic biochemical identification and mass spectrometry identification.The antimicrobial susceptibility of Campylobacter spp.was tested by agar dilution method,and the drug resistance was analyzed.Chi-square test was used to compare count data between groups,and P<0.05 was considered statistically significant.Results A total of 1354 fecal specimens were collected,and the detection rate of Campylobacter was 8.12%,of which,C.jejuni accounted for 82.73%and C.coli accounted for 17.27%.The resistance rates of Campylobacter to nalidixic acid,ciprofloxacin and tetracycline were the highest(73.64%),and the resistance rates of Campylobacter to nalidixic acid,ciprofloxacin and tetracycline in 2019 were higher than those in 2018 and 2021.The resistance rates of C.coli to erythromycin,azithromycin,gentamicin,streptomycin,tyrithromycin and clindamycin were higher than those of C.jejuni.The multi-drug resistance rate of C.jejuni was 36.26%,and the resistance spectrum of C.jejuni was triple drug resistant to fluoroquinolones,tetracyclines and chloramphenicols.The multi-drug resistance rate of C.coli was 57.89%,and the resistance spectrum of C.coli was quintuple drug resistant to fluoroquinolones,tetracyclines,aminoglycosides,macrolides and lincoamides.Conclusions Campylobacter resistance of diarrhea cases in Fengtai District of Beijing is serious.Surveillance,prevention and control of Campylobacter should be strengthened.
