老年慢阻肺合并呼吸衰竭患者的无创正压通气结合电子支气管镜肺泡灌洗治疗作用 被引量:3

Effect of Non-invasive Positive Pressure Ventilation Combined with Electronic Bronchoscopic Alveolar Lavage in Elderly Patients with COPD and Respiratory Failure

摘要 目的分析探讨双水平正压通气(bi-level positive airway pressure,BiPAP)呼吸机与电子支气管镜肺泡灌洗联合治疗老年慢阻肺慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)合并呼吸衰竭患者的临床价值。方法选取2020年2月—2023年1月济南市第三人民医院收治的120例老年慢阻肺合并呼吸衰竭患者为研究对象,按照随机数表法分为对照组和观察组,每组60例,对照组接受单纯无创正压通气治疗,观察组接受无创正压通气结合电子支气管镜肺泡灌洗治疗,比较两组治疗效果。结果治疗后,观察组的pH值、动脉血氧分压、动脉血氧饱和度明显高于对照组,差异有统计学意义(P<0.05),动脉二氧化碳分压(43.97±5.60)mmHg低于对照组,差异有统计学意义(t=10.722,P<0.05);治疗前,最大自主通气量、气道阻力等指标对比,差异无统计学意义(P>0.05);治疗后,观察组最大自主通气量高于对照组,气道阻力明显低于对照组,差异有统计学意义(P<0.05)。讨论使用无创正压通气结合电子支气管镜肺泡灌洗治疗为老年慢阻肺合并呼吸衰竭患者进行治疗可帮助改善患者的血气指标,可促进患者康复。 Objective To investigate the clinical value of b,i-level positive airway pressure(BiPAP)ventilator com-bined with electronic bronchoscopic alveolar lavage in the treatment of elderly patients with chronic obstructive pulmo-nary disease(COPD)complicated with respiratory failure.Methods From February 2020 to January 2023,120 elderly patients with chronic obstructive pulmonary disease and respiratory failure admitted to the Third People's Hospital of Jinan City were selected as the research objects.They were divided into control group and observation group by ran-dom number table method,with 60 cases in each group.The control group was treated with simple non-invasive posi-tive pressure ventilation,while the observation group was treated with non-invasive positive pressure ventilation com-bined with electronic bronchoscopy alveolar lavage.The therapeutic effects of the two groups were compared.Results After treatment,the pH value,arterial partial pressure of oxygen and arterial oxygen saturation in the observation group were significantly higher than those in the control group,the difference was statistically significant(P<0.05),and the arterial partial pressure of carbon dioxide(43.97±5.60)mmHg was lower than that in the control group,the dif-ference was statistically significant(t=10.722,P<0.05).There was no significant difference in the maximum spontane-ous ventilation volume,airway resistance and other indicators before treatment(P>0.05).After treatment,the maxi-mum autonomous ventilation volume of the observation group was higher than that of the control group,and the airway resistance was significantly lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion The use of non-invasive positive pressure ventilation combined with electronic bronchoscopic alveolar la-vage for the treatment of elderly patients with COPD complicated with respiratory failure can help improve the blood gas indexes of patients and promote the rehabilitation of patients.
