摘要
目的评估温控射频治疗对育龄期女性压力性尿失禁(SUI)患者的疗效。方法选择2020年11月至2022年5月于浙江大学医学院附属妇产科医院盆底疾病专科门诊就诊的25例轻至中度SUI患者予以射频治疗,分析治疗前后患者1h尿垫试验结果及国际尿失禁咨询委员会尿失禁问卷简表(ICIQ-SF)、盆腔器官脱垂-尿失禁性生活问卷简表(PISQ-12)的变化情况,并采用整体印象改善评分量表(PGI-I)评价治疗后患者主观疗效。结果射频治疗前后,1h尿垫试验漏尿量分别为(4.96±2.76)g和(2.27±1.98)g,治疗后漏尿量较前显著降低(P<0.05);射频治疗前后患者ICIQ-SF总分分别为(8.72±3.09)分和(6.00±3.16)分,治疗后ICIQ-SF总分较前显著降低(P<0.05),ICIQ-SF漏尿频率、漏尿量、尿失禁对生活影响三个分项得分亦较前显著降低(P<0.05),而PISQ-12总分及伴侣因素分项评分较治疗前显著增加(P<0.05);射频治疗后客观和主观即时有效率分别为60%和88%。结论本研究的结果表明,温控射频治疗是一种安全、有效的物理治疗方法,能够显著改善育龄期女性SUI患者的尿失禁症状、提高生活质量和性功能水平。
Objective This study aimed to evaluate the effectiveness of radiofrequency treatment in managing stress urinary incontinence(SUI)among women of childbearing age.Methods Twenty-five patients with mild to moderate SUI admitted to the specialized clinic of pelvic floor diseases in the Women’s Hospital School of Medicine Zhejiang University from November 2020 to May 2022 were recruited to receive radiofrequency treatment.This study evaluated the results of the 1-hour pad weight test,International Consultation on Incontinence Questionnaire-Short Form(ICIQ-SF)scores,Pelvic Organ Prolapsed-Urinary Incontinence Sexual Questionnaire-12(PISQ-12)scores before and after treatment,and the Patient Global Impression of Improvement(PGI-I)was used to evaluate the subjective effectiveness after treatment.Results All 25 patients completed the experiment.Before and after radiofrequency treatment,the urine leakage volume of 1-h pad weight test were(4.96±2.76)g and(2.27±1.98)g,respectively,and there was a signifcant decrease in urine leakage volume after treatment(P<0.05).The total ICIQ-SF scores before and after radiofrequency treatment were(8.72±3.09)and(6.00±3.16)points,respectively,and there was a signifcant decrease after treatment(P<0.05).The urine leakage frequency scores,urine leakage quantity scores and interference of urine incontinence on life scores of the ICIQ-SF also showed a signifcantly decreased after treatment(P<0.05).The total scores and the score of partner related factors of the PISQ-12 were signifcantly increased after treatment(P<0.05).The objective and subjective timely effective rates reached 60%and 88%,respectively.Conclusion This study showed that radiofrequency treatment,as a physical therapy,is safe and effective in relieving the symptoms of urinary incontinence,improving the quality of life,and enhancing the sexual function among women of childbearing age.
