摘要
目的:检索维持性血液透析病人营养管理相关文献,并进行证据汇总。方法:计算机检索国际指南协作网、英国国家卫生与临床优化研究所、美国国立指南库、加拿大安大略医学会、苏格兰院际间指南网、新西兰指南协作网、中华医学会期刊数据库、中国知网、万方数据库、the Cochrane Library、BMJ Best Practice、国际肾脏病学会、美国肾脏病学会、美国国家肾脏基金会、英国肾脏病协会、国际营养和代谢协会、欧洲临床和营养代谢学会等,检索时限为2012年6月-2022年6月。经过文献纳入、评价、证据提取及汇总,形成证据总结。结果:共纳入文献11篇,包括指南3篇、专家共识4篇、系统评价4篇,从营养支持团队、营养评估与监测、营养素摄入、营养支持治疗、透析期间进食护理、健康教育及随访6个方面形成24条维持性血液透析病人营养管理的最佳证据。结论:最佳证据可为维持性血液透析病人营养管理提供循证依据,在今后临床应用中应结合临床情境及病人实际情况为病人制订个体化的营养管理处方。
Objective:To search the literatures related to nutrition management of maintenance hemodialysis patients and summarize the evidence.Methods:The related literatures were retrieved from the International Guidelines Collaborative Network,the National Institute of Health and Clinical Optimization of the United Kingdom,the National Guidelines Library of the United States,the Ontario Medical Association of Canada,the Scottish Interhospital Guidelines Network,the New Zealand Guidelines Collaborative Network,the Journal Database of the Chinese Medical Association,CNKI,Wanfang database,the Cochrane Library,BMJ Best Practice,the International Society of Nephrology,the American Society of Nephrology,the National Kidney Foundation of the United States,the database of the British Association of Nephrology,the International Association of Nutrition and Metabolism,and the European Society of Clinical and Nutritional Metabolism,the search period was from January 2012 to February 2022.After literature inclusion,evaluation,evidence extraction and summary,evidence summary is formed.Results:A total of 11 papers were included,including 3 guidelines,4 expert consensus articles and 4 systematic reviews,which formed 24 best evidences for nutrition management of maintenance hemodialysis patients from 6 aspects:nutrition support team,nutrition assessment and monitoring,nutrient intake,nutrition support treatment,feeding care during dialysis,health education and follow-up.Conclusions:The best evidence can provide an evidence-based basis for nutritional management of maintenance hemodialysis patients.In the future clinical application,individualized nutritional management prescriptions should be formulated for patients according to the clinical situation and the actual situation of patients.
