摘要
目的 探讨术前不同血浆纤维蛋白原(fibrinogen,FIB)联合中性粒细胞与淋巴细胞比值(neutrophil-lymphocyte ratio,NLR)评分(F-NLR评分)在子宫内膜癌(endometrial cancer,EC)手术患者预后评估中的意义。方法回顾性分析2012年1月至2017年12月在浙江大学医学院附属金华医院妇科接受EC手术经病理确诊的281例患者的临床资料,根据受试者工作特征曲线(receiver operating characteristic curve,ROC)确定FIB和NLR的最佳临界值,将其分为F-NLR评分0、1、2分组;比较不同F-NLR评分患者临床病理资料以及术后5年总体生存率和无病生存率的差异。单因素及多因素回归分析影响EC患者预后的危险因素。结果 单因素分析结果显示:淋巴结转移、国际妇产科联盟(FIGO)分期、肌层浸润深度、NLR、FIB及F-NLR是患者预后不良的影响因素(P<0.05),多因素分析结果显示:高F-NLR评分是患者预后不良的独立危险因素。高F-NLR评分(2分组)预后差于低FNLR评分0、1分组(P<0.05)。结论 术前高F-NLR评分是影响EC手术患者预后的独立危险因素,可将其作为评估EC手术患者预后的有效指标。
Objective To evaluate the significance of F-NLR score in the prognosis of patients with endometrial cancer after surgery.Methods The clinical data of 281 patients with endometrial cancer were analyzed retrospectively.The patients were divided into three gro(F-NLR score O,1,2 groups)according to the values of NLR and FIB defined by the receiver operating characteristic curve(ROC).The clinicopathological data with different F-NLR scores,5-year overall survival rate after surgery and 5-year disease-free survival rate were compared.The risk factors influencing the prognosis of patients with endometrial cancer was analyzed by univariate and multi-univariate regression analysis.Results The results of univariate analysis revealed that lymph node metastasis,FIGO stage,the depth of myometrial invasion,NLR,FIB,and F-NLR score were associated with the prognosis(P<0.05),while the results of multivariate analysis showed that FNLR was an independent risk factor for poor prognosis.The prognosis of high F-NLR score was significantly worse than that of low F-NLR score(P<0.05).Conclusion Preoperative high F-NLR score is an independent risk factor affecting the prognosis of patients with endometrial cancer after surgery,it can be used as an effective index to evaluate the prognosis of patients with endometrial cancer.
