溶血对LC-MS/MS检测血浆中儿茶酚胺的影响

摘要 目的探讨溶血对液相色谱串联质谱(LC-MS/MS)检测血浆中儿茶酚胺及其代谢物的影响,为医学检验实验室检测血浆中儿茶酚胺及代谢物提供指导和参考。方法分别采集30名健康志愿者静脉血,静脉血分别制作成不同程度的溶血标本,并建立溶血液样本模型。用实验室自建的高效LC-MS/MS法分别测定溶血前后血液样本中儿茶酚胺水平,对结果进行双侧配对t检验。结果溶血对肾上腺素、多巴胺的检测结果差异有统计学意义(P<0.05),其中样本溶血后肾上腺素的检测结果明显降低,多巴胺的检测结果明显升高,且多巴胺在中、重度溶血时升高尤其明显,与轻度溶血组间差异有统计学意义(P<0.01);除了3-甲氧基酪胺在溶血前后均无法测出外,溶血对去甲肾上腺素、去甲变肾上腺素、变肾上腺素的检测结果差异无统计学意义(P>0.05)。结论溶血对于肾上腺素、多巴胺有较大的影响,临床采集血液样本时应严格遵守操作规程,防止溶血现象发生,从而确保检验结果的准确性。 Objective To explore the effect of hemolysis on the determination of Catecholamine and its metabolites in plasma by liquid chromatography tandem mass spectrometry(LC-MS/MS),and to provide guidance and reference for medical laboratory in the detection of Catecholamine and its metabolites in plasma.Methods Collect venous blood from 30 healthy volunteers,and make hemolytic samples of varying degrees of venous blood,and establish hemolytic sample models.The levels of Catecholamine in blood samples before and after hemolysis were measured by the self-developed high-performance LC-MS/MS.Results The detection results of adrenaline and dopamine in hemolysis were statistically significant(P<0.05).After sample hemolysis,the detection results of adrenaline decreased significantly,while the detection results of dopamine increased significantly.The increase in dopamine was particularly significant in moderate and severe hemolysis,with a significant difference compared to the mild hemolysis group(P<0.01).Except that 3-neneneba methoxy tyramine could not be detected before and after hemolysis,hemolysis had no statistical significance on the detection results of norepinephrine,normetanephrine and metanephrine(P>0.05).Conclusions Hemolysis has a significant impact on adrenaline and dopamine.When collecting blood samples in clinical practice,strict adherence to operating procedures should be observed to prevent hemolysis and ensure the accuracy of the test results.
