摘要
颅内动脉瘤(IAs)破裂具有很高的致残率和死亡率,如何评估动脉瘤破裂风险高危患者并积极采取恰当的方式治疗尤为重要。目前尚无客观的评估标准,往往根据动脉瘤的部位、大小、形态、血流动力学以及患者存在的高危因素等综合判断。高分辨磁共振成像(HR-MRI)能直接评估动脉瘤壁的厚度、炎症反应、强化程度、血流动力学以及瘤壁渗透率等,还可直接观察瘤内血栓,为评估IAs的破裂风险提供了有效的影像学证据。HR-MRI在IAs的基础研究、影像评估及临床诊断中的应用是未来的重要研究方向。
Intracranial aneurysms(IAs)rupture has a high mortality and disability rate,and it is particularly important to assess patients at high risk for aneurysm rupture and to treat them in an active and appropriate manner.There is no objective assessment standard so far,and it is often comprehensively assessed based on a combination of the location,size,morphology,hemodynamics of the aneurysm,and the presence of high-risk factors in the patient.The application of high resolution magnetic resonance imaging(HR-MRI)in clinical practice can directly assess the characteristics of aneurysm wall thickness,inflammatory response,degree of enhancement,hemodynamics,and aneurysm wall permeability,as well as directly observe intraaneurysm thrombus,providing effective imaging evidence for assessing the risk of rupture of IAs.The application of HR-MRI in the basic research,imaging evaluation and clinical diagnosis of IAs is an important research direction for the future.
