摘要
目的:探讨预防性护理在老年股骨颈骨折患者术后下肢深静脉血栓形成预防中的效果。方法:选取2018年1月—2023年1月苏州市第九人民医院骨关节科收治的102例老年股骨颈骨折患者作为研究对象,采用随机数表法将患者分为观察组和对照组,各51例。对照组给予常规护理,观察组在常规护理基础上给予预防性护理,比较两组临床指标,干预前后股静脉峰流速和血流平均速度、凝血功能及并发症发生情况。结果:观察组首次下床时间早于对照组,住院时间短于对照组,干预后股静脉峰流速、血流平均速度均高于对照组,凝血酶原时间(protothrombin time,PT)、活化部分凝血活酶时间(activated partial thromboplastin time,APTT)均长于对照组,血浆纤维蛋白原(fibrinogen,FIB)低于对照组,并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:在老年股骨颈骨折患者术后预防下肢深静脉血栓形成的护理中,预防性护理的应用能加快康复速度,加快股静脉峰流速和血流平均速度,降低下肢深静脉血栓形成等并发症发生率。
Objective:To analyze the effect of preventive nursing in the prevention of lower limb deep vein thrombosis in elderly patients with femoral neck fractures after surgery.Method:A total of 102 elderly patients with femoral neck fractures who admitted to the Osteoarthritis Department of Suzhou Ninth People's Hospital from January 2018 to January 2023 were selected as the research subjects,the patients were randomly divided into the observation group and the control group according to the random number table method,with 51 patients in each group.The control group was given routine nursing,while the observation group was given preventive nursing on the basis of routine nursing.The clinical indexes,peak flow velocity of femoral vein,average blood flow velocity and coagulation function before and after intervention,and complications were compared between two groups.Result:The first time to get out of bed in the observation group were earlier than those in the control group,hospitalization time in the observation group was shorter than that in the control group,the peak flow velocity of femoral vein,average blood flow velocity after intervention were higher than those in the control group,prothrombin time(PT)and activated partial thromboplastin time(APTT)after intervention were longer than those in the control group,plasma fibrinogen(FIB)was lower than the control group,the incidence of complications was lower than that in the control group,the differences were statistically significant(P<0.05).Conclusion:In the nursing of postoperative prevention of lower limb deep vein thrombosis in elderly patients with femoral neck fracture,the application of preventive nursing can accelerate the recovery speed,accelerate the peak flow velocity of femoral vein and average blood flow velocity,and reduce the incidence of complications such as lower limb deep vein thrombosis.
