磁共振血管成像与多螺旋CT诊断缺血性脑卒中的价值分析 被引量:2

Value Analysis of Magnetic Resonance Angiography and Multispiral CT in the Diagnosis of Ischemic Stroke

摘要 目的:研究磁共振血管成像(MRA)和多层螺旋CT检查在缺血性脑卒中诊断中的价值。方法:选取我院在2021年10月至2023年5月间收治的疑似缺血性脑卒中(acuteischemicstroke,AIS)患者90例为研究对象,均进行DSA检查,确诊AIS的有68例。对90例患者均进行磁共振血管成像检查和多层螺旋CT检查,对比两种方法的诊断效能。结果:MRA检查对AIS的诊断准确率、特异度、灵敏度均高于多层螺旋CT检查的诊断准确率、特异度和灵敏度(P<0.05);对于颈动脉狭窄程度的诊断,对于中度及以上狭窄的诊断准确率、灵敏度和特异度,MRA均高于多层螺旋CT(P<0.05);建立ROC曲线,MRA、多层螺旋CT、MRA联合多层螺旋CT诊断AIS的AUC值分别为0.856、0.755、0.912。结论:与多层螺旋CT检查相比,磁共振血管成像对AIS以及颈动脉狭窄程度的诊断价值更高,并且两种方法可优势互补,联合应用能提高对AIS的诊断价值。 Objective:To investigate the value of magnetic resonance angiography(MRA) and multislice CT examination in the diagnosis of ischemic stroke.Methods:90 patients suspected of acute ischemic stroke(AIS) admitted to our hospital from October 2021 to May 2023 were selected,and DSA was performed to confirm the diagnosis,with 68 patients of AIS confirmed.Magnetic resonance angiography and multi-slice spiral CT were performed on 90 patients to compare the diagnostic efficacy of the two methods.Results:The diagnostic accuracy,specificity and sensitivity of MRA for AIS were higher than those of multi-splice spiral CT(P<0.05);for the diagnosis of carotid stenosis,the accuracy,sensitivity and specificity of MRA in moderate stenosis and above were higher than that of multi-slice spiral CT.(P<0.05);ROC curve was established,the AUC values of MRA,multi-slice spiral CT and MRA combined with multi-slice spiral CT were 0.856,0.755 and 0.912,respectively.Conclusion:Compared with multisplice spiral CT,MRA has a higher diagnostic value for AIS and carotid artery stenosis,and the two methods can complement each other,and the combined application can improve the diagnostic value of AIS.
