摘要
目的:分析直肠肿瘤患者行手套法经肛微创手术与经肛内镜微创手术的治疗效果。方法:选取2019年3月—2021年3月海南省琼海市人民医院收治的直肠肿瘤患者120例为研究对象,根据随机数字表法分为研究组和对照组,各60例。研究组行手套法经肛微创手术治疗,对照组行肛门内镜微创手术治疗,比较两组患者手术恢复结果、术后肛门功能、术后并发症发生情况、肿瘤复发转移情况。结果:研究组手术时间短于对照组,治疗费用少于对照组,差异有统计学意义(P<0.05);两组患者肛门失禁Wexner评分比较,差异无统计学意义(P>0.05);两组患者术后并发症发生率比较,差异无统计学意义(P>0.05);研究组有2例患者在术后12个月内肿瘤复发,对照组2例患者在术后6个月经MRI检查示直肠系膜淋巴结转移。结论:手套法经肛微创手术与经肛内镜微创手术治疗直肠肿瘤效果相当,且安全性好,手术时间短,治疗费用少。
Objective:To analyze the therapeutic effect of gloves minimally invasive transanal surgery and transanal endoscopic minimally invasive surgery in patients with rectal tumor.Methods:A total of one hundred and twenty patients with rectal tumor admitted to Hainan Province Qionghai People's Hospital from March 2019 to March 2021 were selected as the study objects,and were divided into study group and control group according to random number table method,with sixty cases in each group.The study group underwent minimally invasive transanal surgery with glove method,while the control group underwent endoscopic minimally invasive anal surgery.The results of surgical recovery,postoperative anal function,postoperative complications,and tumor recurrence and metastasis were compared between the two groups.Results:The operation time of the study group was shorter than that of the control group,and the treatment cost was lower,the difference was statistically significant(P<0.05);There was no significant difference in Wexner score between the two groups(P>0.05);There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05);In the study group,2 patients had tumor recurrence within 12 months after surgery,and in the control group,2 patients showed mesenteric lymph node metastasis by MRI examination 6 months after surgery.Conclusion:Glove minimally invasive transanal surgery is comparable to endoscopic transanal minimally invasive surgery for the treatment of rectal tumors,and has good safety,short operation time and low treatment cost.
