摘要
目的 分析带状疱疹及带状疱疹后神经痛(postherpeticneuralgia,PHN)病人心理痛苦现状,并根据患者现状分析影响患者心理痛苦的因素。方法 选取某市三级甲等医院2021年6月-2022年9月疼痛康复科收治的91例带状疱疹患者作为研究对象,根据患者是否发生PHN将患者分为PHN患者及无PHN患者,采用本院自制量表对患者一般资料进行调查,采用心理痛苦温度计对患者进行问卷调查,并根据患者现状分析导致患者产生心理痛苦的影响因素。结果 本次研究共38例患者发生PHN,发生率为41.75%。PHN组与无PHN组心理痛苦平均得分分别为(6.24±2.98)分、(3.59±1.21)分,两组差异显著(P<0.05)。单因素分析结果显示:性别、年龄、负面情绪、糖尿病、皮疹面积、急性期疼痛程度是PHN患者心理痛苦的影响因素(P<0.05)。Logistic回归分析结果显示:年龄、负面情绪、皮疹面积、急性期疼痛程度是PHN患者心理痛苦的主要影响因素。结论 PHN患者心理痛苦的发生率较高,护理人员应对高龄、负性情绪高、皮疹面积大以及急性期疼痛程度高的患者给予更多的关注,帮助其解决相关情绪问题,减少心理痛苦的发生。
Objective To analyze status of psychological pain in patients with shingles and postherpeticneuralgia(PHN)and influencing factors of psychological pain according to status of patients.Methods The paper chose 91 patients with shingles from June 2021 to September 2022 in pain rehabilitation department of a tertiary first-class hospital in a city,and divided them into PHN patients and non PHN patients according to whether the patients had PHN.General data of patients were investigated with self-designed scale of the hospital and patients were investigated with psychological pain thermometer and influencing factors for psychological pain were analyzed according to current situation.Results There were 38 patients with PHN in the study,with incidence rate of 41.75%.Average psychological pain scores of PHN group and non PHN group were(6.24±2.98)and(3.59±1.21)respectively,with significant difference between two groups(P<0.05).Univariate analysis showed,gender,age,negative emotions,diabetes,rash area and acute pain were influencing factors of psychological pain of PHN patients(P<0.05).Logistic regression analysis showed,age,negative emotions,rash area and acute pain level were main influencing factors for psychological pain of PHN patients.Conclusion Incidence of psychological pain of PHN patients is relatively high,nursing staff should pay more attention to patients with advanced age,high negative emotions,large rash area and high acute pain level,help them solve related emotional problems and reduce incidence of psychological pain.
