标准抗Hp治疗方案对Hp阳性冠心病患者血脂代谢及血清MMP-9和ICAM-1水平的影响

摘要 目的探究标准抗幽门螺杆菌(Hp)治疗方案对Hp阳性冠心病(CHD)患者血脂代谢及血清基质金属蛋白酶(MMP-9)、细胞间黏附分子(ICAM-1)水平的影响。方法选择2021年1月至2023年1月来院就诊的Hp阳性CHD患者112例,依照随机数表法分成观察组(n=56)和对照组(n=56)。对照组采用常规治疗方式,观察组在此基础上进行抗Hp治疗。比较两组治疗后临床效果,比较两组治疗前后的血脂代谢指标、MMP-9、ICAM-1及内皮功能[内皮素-1(ET-1)、一氧化氮(NO)]、同型半胱氨酸(Hcy),并记录两组患者的不良反应。结果观察组及对照组治疗后的总有效率为94.64%、82.14%,差异有统计学意义(P<0.05);治疗后,观察组的TC、TG、LDL-C、MMP-9、ICAM-1、NO水平分别为(3.52±0.95)mmol/L、(1.54±0.52)mmol/L、(2.56±0.71)mmol/L、(83.22±19.31)μg/L、(344.15±78.37)μg/L、(62.78±7.12)μmol/L,均低于对照组的(4.17±1.12)mmol/L、(2.03±0.48)mmol/L、(2.94±0.63)mmol/L、(106.45±22.68)μg/L、(461.54±86.26)μg/L、(57.25±7.34)μmol/L(P<0.05);观察组的HDL-C、Hcy、ET-1水平分别为(1.37±0.42)mmol/L、(7.19±1.38)μmol/L、(46.31±5.26)pg/mL,均高于对照组的(1.21±0.34)mmol/L、(16.52±2.07)μmol/L、(49.05±5.33)pg/mL(P<0.05);治疗期间,两组不良反应发生率分别为17.86%、14.29%,差异无统计学意义(P>0.05)。结论标准抗Hp治疗方案能够提高Hp阳性CHD患者的治疗效果,改善其血脂及黏附分子水平,减少对血管内皮的损伤,且具有一定安全性。 Objective To explore the effects of standard anti-Helicobacter pylori(Hp)treatment regimen on levels of blood lipid metabolism indexes,serum matrix metalloproteinase-9(MMP-9)and intercellular adhesion molecule-1(ICAM-1)in patients with HP-positive coronary heart disease(CHD).Methods A total of 112 patients with HP-positive CHD treated in the hospital were enrolled between January 2021 and January 2023.According to random number table method,they were divided into observation group(n=56,anti-Hp treatment on bais of control group)and control group(n=56,routine treatment).The clinical effect after treatment,blood lipid metabolism indexes,MMP-9,ICAM-1,endothelial function[endothelin-1(ET-1),nitric oxide(NO)]and homocysteine(Hcy)were compared between the two groups before and after treatment.And the adverse reactions in both groups were recorded.Results After treatment,differences in total response rate between observation group and control group were statistically significant(94.64%vs 82.14%,P<0.05).After treatment,levels of TC,TG,LDL-C,MMP-9,ICAM-1 and NO in observation group were(3.52±0.95)mmol/L,(1.54±0.52)mmol/L,(2.56±0.71)mmol/L,(83.22±19.31)μg/L,(344.15±78.37)μg/L and(62.78±7.12)μmol/L,lower than those in control group[(4.17±1.12)mmol/L,(2.03±0.48)mmol/L,(2.94±0.63)mmol/L,(106.45±22.68)μg/L,(461.54±86.26)μg/L,(57.25±7.34)μmol/L,P<0.05],while levels of HDL-C,Hcy and ET-1 were(1.37±0.42)mmol/L,(7.19±1.38)μmol/L and(46.31±5.26)pg/mL,higher than those in control group[(1.21±0.34)mmol/L,(16.52±2.07)μmol/L,(49.05±5.33)pg/mL,P<0.05].During treatment,difference in the incidence of adverse reactions between the two groups was not statistically significant(17.86%vs 14.29%,P<0.05).Conclusion Standard anti-HP treatment regimen can improve curative effect,blood lipid and adhesion molecules,and reduce vascular endothelial injury in patients with HP-positive CHD,with certain safety.
