摘要
目的:研究CT结合血清甲胎蛋白(AFP)与癌胚抗原(CEA)在肝硬化结节与小肝癌患者中的鉴别作用。方法:选取济宁市兖州区人民医院2018年1月—2022年12月收治的小肝癌患者52例为观察组,同期选择我院诊治的肝硬化结节患者38例为对照组。均开展CT灌注扫描,且抽取患者的空腹静脉血,比较两组患者的血流量(BF)、血容量(BV)、对比剂平均通过时间(MTT)、肝动脉分数(HAF)参数变化,比较两组的AFP与CEA水平变化情况。结果:观察组BF、BV与HAF均高于对照组(P<0.05);观察组AFP、CEA高于对照组(P<0.05)。CT灌注、AFP、CEA检查肝硬化结节与小肝癌的灵敏度、特异度与三者联合比较明显较低(P<0.05)。结论:CT灌注参数、血清AFP、CEA联合有助于鉴别肝硬化结节与小肝癌疾病,且联合诊断具有较高的灵敏度和特异度。
Objective To investigate the role of CT combined with serum alpha-fetoprotein(AFP)and carcinoembryonic antigen(CEA)in the differentiation of cirrhotic nodules and small liver cancer.Methods 52 patients with small liver cancer treated in Yanzhou District People's Hospital of Jining City from January 2018 to December 2022 were selected as the observation group,and 38 patients with cirrhosis nodular treated in our hospital during the same period were selected as the control group.CT perfusion scan was performed in all patients,and fasting venous blood was collected from the patients,and blood flow(BF),blood volume(BV),mean transit time(MTT)and fraction of the hepatic artery(HAF)parameters of the two groups were compared,and AFP and CEA levels were compared between the two groups.Results BF,BV and HAF in observation group were higher than those in control group(P<0.05).AFPand CEAin observation group were higher than those in control group(P<0.05).The sensitivity and specificity of CT perfusion,AFP and CEA in the detection of liver cirrhosis nodules and small liver cancer were significantly lower than that of the combination of CT perfusion,AFP and CEA(P<0.05).Conclusion The combination of CT perfusion parameters,serumAFP and CEAis helpful to distinguish liver cirrhosis nodules from small liver cancer diseases,and the combined diagnosis has high sensitivity and specificity.
