孕妇血清维生素D水平与胰岛素抵抗、炎症反应的相关性 被引量:3

Correlation of serum vitamin D level with insulin resistance and inflammatory response in pregnant women

摘要 目的探讨孕妇血清维生素D(Vit D)水平变化及其与胰岛素抵抗(IR)、炎症反应的相关性。方法选择2020年1月至2021年12月江苏省无锡市锡山人民医院(以下简称“我院”)产检的53例妊娠糖尿病(GDM)孕妇为试验组,另选取同期我院产检的53名正常孕妇为对照组。记录两组临床资料;比较两组空腹血糖(FBG)、空腹胰岛素(FINS)、稳态模型评估法-胰岛素抵抗(HOMA-IR),总胆固醇(TC)、甘油三酯(TG)、25羟维生素D[25(OH)D]、超敏C-反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)水平;血清25(OH)D与各指标之间的相关性采用Pearson相关性分析。结果试验组25(OH)D水平低于对照组,FBG、FINS、TG、HOMA-IR、hs-CRP、TNF-α及IL-6水平高于对照组,差异有统计学意义(P<0.05)。血清25(OH)D水平与FBG、FINS、TG、HOMA-IR、CRP、TNF-α、IL-6呈负相关(r<0,P<0.05)。结论与健康孕妇比较,GDM孕妇体内Vit D水平明显降低,其水平下降可能是引发炎症反应、IR及糖脂代谢异常等的重要因素之一,临床应加强对GDM孕妇Vit D水平变化情况的重视程度。 Objective To explore the changes of serum vitamin D(Vit D)level and its correlation with insulin resistance(IR)and inflammatory response in pregnancy.Methods A total of 53 pregnant women with gestational diabetes mellitus(GDM)were selected as experimental group from the Xishan People’s Hospital of Wuxi,Jiangsu Province(hereinafter referred to as“our hospital”)from January 2020 to December 2021,another 53 normal pregnant women in our hospital during the same period were selected as control group.The clinical data in both groups were recorded;the levels of fasting blood glucose(FBG),fasting insulin(FINS),Homeostasis model evaluation method-insulin resistance(HOMA-IR),total cholesterol(TC),triglyceride(TG),25-hydroxyvitamin D(25[OH]D),hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),and interleukin-6(IL-6)in both groups were compared;the correlation between serum 25(OH)D and each index was analyzed by Pearson correlation analysis.Results The level of 25(OH)D in experimental group was lower than that in control group,the levels of FBG,FINS,TG,HOMA-IR,hs-CRP,TNF-α,and IL-6 in experimental group were higher than those in control group,and the differences were statistically significant(P<0.05).The level of serum 25(OH)D was negatively correlated with FBG,FINS,TG,HOMA-IR,CRP,TNF-α,and IL-6(r<1,P<0.05).Conclusion Compared with healthy pregnant women,the level of Vit D in GDM pregnant is significantly decreased,which may be one of the important factors causing inflammation, IR and abnormal glucose and lipid metabolism, etc. Clinical attention should be paid to the changes of Vit D level in GDM pregnant.
