摘要
目的 探讨胃癌患者行腹腔镜胃癌根治术后发生胃瘫综合征(PGS)的危险因素。方法 收集326例行腹腔镜胃癌根治术的胃癌患者的临床资料,对可能影响PGS发生的危险因素进行分析。结果 326例胃癌术后患者,发生PGS 18例。年龄≥65岁、手术方式为胃空肠吻合、术前有幽门梗阻、术前有低蛋白血症、手术时间≥4 h、围手术期有高糖血症的胃癌患者PGS发生率分别高于年龄﹤65岁、手术方式为胃十二指肠吻合、术前无幽门梗阻、术前无低蛋白血症、手术时间﹤4 h、围手术期无高糖血症的胃癌患者,差异均有统计学意义(P﹤0.05)。结论 具有临床危险因素的胃癌患者行腹腔镜胃癌根治术后PGS发生率增加,且PGS的发生会影响患者预后。做好围手术期的处理工作对预防PGS的发生有重要意义。
Objective To investigate the risk factors of postsurgical gastroparesis syndrome(PGS)in patients with gastric cancer after laparoscopic radical gastrectomy.Method The clinical data of 326 patients with gastric cancer treat-ed by laparoscopic radical gastrectomy were collected and the risk factors that might affect the occurrence of PGS were analyzed.Result PGS occurred in 18 of 326 patients with gastric cancer after surgery.The incidences of PGS in gastric cancer patients aged≥65 years,who underwent gastrojejunal anastomosis,had pyloric obstruction before surgery,had hy-poproteinemia before surgery,operation time≥4 h and had perioperative hyperglycaemia were higher than those of pa-tients aged<65 years,who underwent gastroduodenal anastomosis,had no pyloric obstruction before surgery,had no hy-poproteinemia before surgery,operative time<4 h,and had no perioperative hyperglycemia(P<0.05).Conclusion Pa-tients with gastric cancer with clinical risk factors have an increased probability of PGS after laparoscopic radical gastrec-tomy,and the occurrence of PGS will influence the prognosis of patients.It is of great significance to prevent PGS by making a good control of perioperative management.
