摘要
经皮血管腔内成形术是治疗脑血管病常用的介入治疗方式,其手术种类多,解剖部位精细,编码较为复杂易错。文章结合临床知识、ICD-9-CM-3分类规则和错误案例,总结分析脑血管腔内成形术的术式和编码。脑血管腔内成形术要与脑血管腔内隔绝术相鉴别,脑血管腔内成形术编码集中在00.61-00.65,其中经皮脑血管球囊扩张成形术编码至00.61-00.62,经皮脑血管支架置入成形术编码至00.63-00.65,细目的分类轴心是解剖部位颅外或颅内血管。脑血管覆膜支架置入术是一种腔内隔绝术,编码于39.72。在编码时应当避免按临床医师书写的手术名称直接在编码库中选择名称相近的编码,要坚持遵守手术编码查找步骤,切记要在类目表中核对编码,从而获得正确的ICD-9-CM-3编码。
Percutaneous transluminal Angioplasty is a common interventional therapy used in cerebrovascular diseases.The codes of percutaneous transluminal cerebrovascular angioplasty are complex and fallible because of so many types of surgery and the elaborate anatomy.This article combines clinical knowledge,ICD-9-CM-3 coding rules,and error cases to summarize and analyze the surgical procedures and codes of percutaneous transluminal cerebrovascular angioplasty.It is known that percutaneous transluminal cerebrovascular angioplasty should be differentiated from endovascular stent-graft exclusion surgery by the analysis.The percutaneous transluminal cerebrovascular angioplasty should be coded in 00.61-00.65.Percutaneous balloon dilated angioplasty of cerebral vessels should be coded in 00.61-00.62.Percutaneous stent implantation for cerebral vessels should be coded in 00.63-00.65.The classified axis of detailed categories is extracranial or intracranial vessels.Implantation of a membrane-covered stent in cerebral vessels is a type of endovascular stent-graft exclusion surgery and should be coded in 39.72.It should be avoided to directly select codes with similar surgical names in the code library according to the name of the operation written by the clinician.The rules of surgical coding search steps should be obeyed strictly and the step of checking the codes in the subcategories list should be remembered in order to find the correct ICD-9-CM-3 codes.
