HIF-1α、E-Cadherin、Snail+Slug在宫颈鳞癌中的表达及意义

摘要 目的研究宫颈鳞癌中缺氧诱导因子-1α(HIF-1α)与上皮间充质转化(EMT)相关蛋白E-Cadherin、Snail+Slug的表达及其相关性。方法运用基因芯片免疫组织化学方法检测HIF-1α、E-Cadherin、Snail+Slug在20例正常宫颈组织(NC组)和80例宫颈鳞癌组织(ICC组)中的表达。结果HIF-1α、E-Cadherin、Snail+Slug在ICC组的阳性表达率分别为77.50%(62/80)、67.50%(54/80)、36.25%(29/80),在NC组的阳性表达率分别为5.00%(1/20)、95.00%(19/20)、0(0/20);HIF-1α、E-Cadherin及Snail+Slug在NC组及ICC组中的阳性表达率比较,差异具有统计学意义(P<0.05)。80例宫颈鳞癌患者中,HIF-1α阳性62例(77.50%),E-Cadherin阳性54例(67.50%),Snail+Slug阳性29例(36.25%)。临床分期Ⅱ期患者的HIF-1α阳性率高于临床分期Ⅰ期患者(P<0.05);临床分期Ⅰ期、高+中分化程度、浸润深度<1/2宫颈间质、无淋巴结转移患者的E-Cadherin阳性率分别高于临床分期Ⅱ期、低分化程度、浸润深度≥1/2宫颈间质、有淋巴结转移患者(P<0.05);低分化程度、浸润深度≥1/2宫颈间质患者的HIF-1α、Snail+Slug阳性率分别高于高+中分化程度、浸润深度<1/2宫颈间质患者(P<0.05);有淋巴结转移患者的Snail+Slug阳性率高于无淋巴结转移患者(P<0.05)。相关性分析结果显示,HIF-1α与E-Cadherin的表达呈负相关,与Snail+Slug的表达呈正相关(P<0.05)。结论HIF-1α蛋白的表达水平与宫颈鳞癌的分化程度、浸润深度密切相关。HIF-1α的过表达可能通过EMT途径促进宫颈鳞癌的浸润与转移,联合检测HIF-1α蛋白表达可作为评价患者预后的生物学指标。 Objective To study the expression and correlation of hypoxia inducible factor-1α(HIF-1α)and E-Cadherin,Snail+Slug of epithelial mesenchymal transition(EMT)related proteins in cervical squamous cell carcinoma.Methods The expression of HIF-1α,E-Cadherin and Snail+Slug in 20 cases of normal cervical tissues(NC group)and 80 cases of cervical squamous cell carcinoma(ICC group)were detected by gene chip immunohistochemistry.Results The positive expression rates of HIF-1α,E-Cadherin and Snail+Slug in the ICC group were 77.50%(62/80),67.50%(54/80)and 36.25%(29/80),respectively,and the positive expression rates in the NC group were 5.00%(1/20),95.00%(19/20)and 0(0/20),respectively;the positive expression rates of HIF-1α,E-Cadherin and Snail+Slug in the NC group and the ICC group were significantly different(P<0.05).Among the 80 patients with cervical squamous cell carcinoma,62 cases(77.50%)were positive for HIF-1α,54 cases(67.50%)were positive for E-Cadherin,and 29 cases(36.25%)were positive for Snail+Slug.The positive rate of HIF-1αin patients with clinical stageⅡwas higher than that in patients with clinical stageⅠ(P<0.05);the positive rate of E-Cadherin in patients with clinical stageⅠ,high+moderate differentiation,infiltration depth<1/2 cervical stroma and no lymph node metastasis were higher than those in patients with clinical stageⅡ,low differentiation,infiltration depth≥1/2 cervical stroma and lymph node metastasis(P<0.05);the positive rates of HIF-1αand Snail+Slug in patients with low differentiation and infiltration depth≥1/2 cervical stroma were higher than those in patients with high+moderate differentiation and infiltration depth<1/2 cervical stroma(P<0.05);the positive rate of Snail+Slug in patients with lymph node metastasis was higher than that in patients without lymph node metastasis(P<0.05).The results of correlation analysis showed that HIF-1αwas negatively correlated with the expression of E-Cadherin and positively correlated with the expression of Snail+Slug(P<0.05).Conclusion The expression level of HIF-1αprotein is closely related to the degree of differentiation and depth of invasion of cervical squamous cell carcinoma.The overexpression of HIF-1αmay promote the invasion and metastasis of cervical squamous cell carcinoma through the EMT pathway.The combined detection of HIF-1αprotein expression can be used as a biological indicator to evaluate the prognosis of patients.
