摘要
目的:探究有创机械通气基础上给予大承气汤治疗急性肺损伤/急性呼吸窘迫综合征(ALI/ARDS)的临床疗效及安全性.方法:选取本院2017年12月至2021年12月收治的108例ALI/ARDS患者为研究对象.根据不同治疗方法将其分为观察组(n=56,有创机械通气治疗基础上给予大承气汤治疗)与对照组(n=52,仅给予有创机械通气治疗).比较两组临床疗效、中医证候积分、急性生理与慢性健康状况评分Ⅱ(APACHEⅡ)评分、Murray肺损伤评分、炎性因子及不良反应发生率.结果:观察组总有效率高于对照组,差异有统计学意义(P<0.05).治疗后,观察组中医证候积分、APACHEⅡ评分、Murray肺损伤评分及白细胞介素(IL)-6、肿瘤坏死因子(TNF)-α和C反应蛋白(CRP)水平均低于对照组,差异有统计学意义(P<0.05).观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05).结论:有创机械通气基础上给予大承气汤治疗ALI/ARDS患者疗效显著,具有一定安全性.
Objective:To analyze the clinical efficacy of conventional mechanical ventilation combined with dachengqi decoction in the treatment of acute lung injury/acute respiratory distress syndrome(ALV/ARDS).Methods:A total of 108 ALV/ARDS patients admtted to our hospital from December 2017 to December 2021 were selected in this study.According to different treatment methods,they were divided into observation group(n=56,conventional mechanical ventilation combined with Dachengqi decoction)and control group(n=52,conventional mechanical ventilation only).The clinical efficacy,traditional Chinese medicine(TCM)syndrome scores,acute physiological and chronic health status scoreⅡ(APACHEⅡ),Murray lung injury score,inflammatory factors,and the incidence of adverse reactions were compared between the two groups.Results:The total effective rate of observation group was significantly higher than that of control group(P<0.05).After treatment,the TCM syndrome score,APACHEⅡscore,Murray lung injury score,IL-6,TNF-αand CRP levels in observation group were lower than those in control group(P<0.05).The incidence of adverse reactions in observation group was lower than that in control group,the difference is statistically significant(P<0.05).Conclusion:Dachengqi decoction has a significant effect on ALVARDS patients and certain safety in treating ALV/ARDS patients.
