摘要
目的探讨急性心肌梗死院前急救中预防性应用胺碘酮的价值。方法2021年1月~2022年6月院前急救的急性心肌梗死患者148例,按照随机数字表分为利多卡因组与胺碘酮组各74例,在常规院前急救的基础上分别应用利多卡因及胺碘酮。观察心电监护结果、除颤次数及住院时间,不良反应,预后及复发情况。结果现场抢救前心电监护差异无统计学意义(P>0.05),经过运送抵达医院时胺碘酮组患者呼吸频率、心率、平均动脉压低于利多卡因组,血氧饱和度高于利多卡因组(P<0.05);胺碘酮组除颤次数(1.48±0.52)次,住院时间(10.22±1.67)d少于利多卡因组(2.89±0.83)次,(11.53±1.90)d(P<0.05);胺碘酮组患者抢救成功率91.89%高于利多卡因组78.39%(P<0.05),心肌梗死复发率5.41%低于利多卡因组16.22%(P<0.05);不良反应发生率2.70%低于利多卡因组16.22%(P<0.05)。结论急性心肌梗死院前急救中预防性应用胺碘酮能够提高抢救成功率,降低心肌梗死复发率。
Objective To explore the value of preventive amiodarone in pre-hospital emergency treatment of AMI.Methods From January 2021 to June 2022,148 patients with AMI who received pre-hospital emergency treatment were divided into lidocaine group and amiodarone group(74 cases each)according to the random number table.On the basis of routine pre hospital emergency treatment,lidocaine and amiodarone were used respectively.Outcome measures ECG monitoring results,defibrillation frequency and hospitalization time,adverse reactions,prognosis and recurrence.Results There was no difference in ECG monitoring before on-site rescue(P>0.05).The respiratory rate,heart rate and mean arterial pressure of patients in the amiodarone group were lower than lidocaine group.Blood oxygen saturation was higher than lidocaine group(P<0.05).Amiodarone group defibrillation frequency(1.48±0.52)times,hospitalization time(10.22±1.67)days were less than(2.89±0.83)times and(11.53±1.90)days in lidocaine group(P<0.05).Rescue success 91.89%was higher than that lidocaine group 78.39%(P<0.05).Myocardial infarction recurrence rate 5.41%was lower than 6.22%in lidocaine group(P<0.05).Lidocaine group ADR 2.70%was lower than lidocaine group 16.22%(P<0.05).Conclusion The preventive use of amiodarone in pre-hospital emergency treatment of AMI can improve the success rate of rescue and reduce the recurrence of myocardial infarction.
