超声监测胎儿脑脐血流诊断子痫前期孕妇发生胎儿生长受限价值研究 被引量:6

The Ultrasonic Testing of Cerebral and Umbilical Blood Flow on the Diagnosis of Fetal Growth Restriction for Preeclampsia Pregnant Women

摘要 目的研究超声监测胎儿脑脐血流对子痫前期(PE)孕妇发生胎儿生长受限(FGR)的诊断价值。方法对洛阳市妇幼保健院2021年1月至2022年12月接收的80例PE孕妇的病历资料进行回顾性分析,并根据临床诊断结果,将所有入选者分为对照组(未发生FGR,n=33)与FGR组(合并FGR,n=47),比较两组孕妇的脑脐血流参数、脑胎盘率(CPR),并以临床诊断结果为金标准,分析脑脐血流参数及CPR诊断FGR的价值。结果FGR组大脑中动脉收缩期峰值血流速度与舒张末期血流速度比值(S/D)、阻力指数(RI)、搏动指数(PI)等血流参数均低于对照组,而脐动脉S/D、RI、PI等血流参数均高于对照组,差异有统计学意义(P<0.05);FGR组CPR为(1.27±0.18)低于对照组的(1.74±0.23),差异有统计学意义(P<0.05);脑脐血流参数联合诊断的灵敏度、特异度、准确性、阳性与阴性预测值分别为89.36%、84.85%、88.75%、81.25%、87.50%,对FGR的诊断效能最优,CPR次之。结论脑脐血流参数(S/D、RI、PI)与CPR对PE孕妇发生FGR具有一定的诊断价值,而六项脑脐血流参数联合检测可取得更加显著的诊断效果。 Objective To study the ultrasonic testing of cerebral and umbilical blood flow on the diagnosis of fetal growth restriction(FGR)for preeclampsia(PE)pregnant women.Methods Retrospective analysis of the medical records of 80 PE pregnant women admitted to Luoyang Maternal and Child Health Hospital from January 2021 to December 2022.According to the clinical diagnosis results,women were assigned to control group(n=33,without FGR)and FGR group(n=47,with FGR).Compare the cerebral and umbilical blood flow parameters,cerebro-placental ratio(CPR)between two groups of pregnant women.The clinical diagnosis results were seen as gold standard.Analyze the Brain umbilical blood flow parameters and the value of CPR in diagnosing FGR.Results The cerebral blood flow parameters like Peak Systolic Velocity/End Diastolic Velocity(S/D),resistance index(RI)and pulsatility index(PI)in FGR group were significantly lower than control group;the umbilical blood flow parameters like S/D,RI and PI in FGR group were significantly higher than control group,and the difference was statistically significant(P<0.05);the CPR of the FGR group was(1.27±0.18)lower than that of the control group(1.74±0.23),with a statistically significant difference(P<0.05);The sensitivity,specificity,accuracy,positive and negative predictive values of the combined diagnosis of Brain umbilical blood flow parameters were 89.36%,84.85%,88.75%,81.25%,and 87.50%,respectively.The diagnostic performance of FGR is the best,followed by CPR.Conclusion The combination of cerebral umbilical blood flow parameters(S/D,RI,PI)and CPR has certain diagnostic value for the occurrence of FGR in PE pregnant women,and the combined detection of six cerebral umbilical blood flow parameters can achieve more significant diagnostic results.
