摘要
目的:分析多层螺旋CT(MSCT)平扫及三维重建技术在外伤性髋关节骨折诊断中的应用价值。方法:选取2020年7月—2022年9月于临沂市中心医院就诊的疑似外伤性髋关节骨折患者80例为研究对象,所有患者均行MSCT平扫及三维重建技术检查。以手术结果为“金标准”,分析MSCT平扫及三维重建技术检查结果,另分析MSCT平扫及三维重建技术在外伤性髋关节骨折中的诊断价值及一致性。结果:80例疑似患者经手术明确诊断为外伤性髋关节骨折患者75例,包括前柱骨折19例,后壁骨折15例,后柱骨折13例,复合骨折10例,前壁骨折18例;MSCT平扫检查诊断外伤性髋关节骨折符合率为77.33%(58/75),Kappa检验显示:MSCT平扫与“金标准”的一致性良好(Kappa值=0.724)。MSCT平扫及三维重建技术诊断外伤性髋关节骨折符合率为92.00%(69/75),与MSCT平扫符合率77.33%(58/75)比较,差异有统计学意义(P=0.013),Kappa检验显示:MSCT平扫及三维重建技术与“金标准”的一致性极好(Kappa值=0.936)。结论:MSCT平扫及三维重建技术在外伤性髋关节骨折诊断中应用价值较高,有助于明确骨折位置,为临床进一步诊疗提供可靠依据,值得推广应用。
Objective To analyze the application value of multi-slice spiral CT(MSCT)plain scan and Iterative reconstruction technology in the diagnosis of traumatic hip fracture.Methods Eighty patients with suspected traumatic hip fracture treated in Linyi Central Hospital from July 2020 to September 2022 were selected as the study objects.All patients were examined by MSCT plain scan and three-dimensional reconstruction technology.Taking the surgical results as the"gold standard",the results of MSCT plain scan and 3D reconstruction were analyzed,and the diagnostic value and consistency of MSCT plain scan and 3D reconstruction in traumatic hip fracture were analyzed.Results Of the 80 suspected patients,75 were diagnosed as traumatic hip fracture by operation,including 19 anterior column fractures,15 posterior wall fractures,13 posterior column fractures,10 compound fractures,and 18 anterior wall fractures.The coincidence rate of MSCT plain scan in the diagnosis of traumatic hip fracture was 77.33%(58/75),and Kappa test showed that MSCT plain scan was in good agreement with"gold standard"(Kappa value=0.724).The coincidence rate of MSCT plain scan and three-dimensional reconstruction in the diagnosis of traumatic hip fracture was 92.00%(69/75),compared with MSCT plain scan,the coincidence rate was 77.33%(58/75),and the difference was statistically significant(P=0.013).Kappa test showed that:MSCT plain scan and 3D reconstruction were in excellent agreement with"gold standard"(Kappa value=0.936).Conclusion MSCT plain scan and 3D reconstruction technology have high application value in the diagnosis of traumatic hip fracture,which can help to identify the fracture location and provide a reliable basis for further clinical diagnosis and treatment.
