摘要
目的探讨睾丸平滑肌瘤的临床表现、影像学及病理特点、治疗方式和预后。方法回顾性分析南京医科大学附属苏州医院2例睾丸平滑肌瘤患者的临床资料,其中病例1 B超示左侧睾丸下方实性占位,行左侧睾丸根治性切除术;病例2 B超示右侧阴囊内异常回声肿块,盆腔CT未见明显异常,行右睾丸根治性切除术。术后均行病理检测,并进行随访观察。结果病例1术后病理考虑为平滑肌瘤特征;免疫组化示Vimentin、Desmin、SMA、Calponin、Actin、CD34均呈阳性。病例2术后病理示梭形细胞肿瘤,符合平滑肌瘤特征;免疫组化示Vimentin、Desmin、SMA、Calponin、Actin、CD34均呈阳性。2例患者均顺利完成手术,术后复查均未见复发及转移。结论睾丸平滑肌瘤在临床上罕见,且为良性肿瘤,预后良好。临床表现和影像学检查无特异性,病理诊断结果为金标准,手术根治切除为目前主要的治疗方法。
Objective To investigate the features of clinical performance,imaging,pathology,treatment and prognosis of intratesticular leiomyoma.Methods The clinical data of two patients with intratesticular leiomyoma were analyzed retrospectively.In case one,B-scan ultrasonography showed solid mass below the left testicle and radical excision of the left testicle was performed.In case two,B-scan ultrasonography revealed abnormal echoic mass in the right scrotum,while pelvic CT scan revealed no obvious abnormalities.Radical excision of right testicle was performed and followed by pathological examination and follow-up observation.Results Postoperative pathological return of case one was leiomyoma.Immunohistochemical results:Vimentin,Desmin,SMA,Calponin,Actin and CD34 were positive.Postoperative pathology of case two revealed spindle cell tumor,consistent with leiomyoma.Immunohistochemical results:Vimentin,Desmin,SMA,Calponin,Actin and CD34 were positive.No recurrence or metastasis was observed during follow-up.Conclusions Leiomyoma of the testis is a rare and benign tumor with no typical performance,and postoperative pathology is the gold standard for diagnosis.Radical surgical excision is the main treatment for intratesticular leiomyoma.
