摘要
目的 分析急性上呼吸道感染(Acute upper respiratory tractinfection, AURI)儿童肺炎链球菌(Streptococcus pneumoniae,Sp)和流感嗜血杆菌(Haemophilus influenza,Hi)携带状况。方法 2021年3月-2022年6月在昆明市儿童医院采集0-16岁AURI就诊儿童咽拭子标本,采用实时荧光定量聚合酶链反应检测Sp和Hi核酸,分析检测阳性率。结果 在1 021名调查对象中,Sp和Hi阳性率分别为20.86%和19.00%,Sp和Hi同时阳性率为6.86%。0-2岁、3-5岁、6-16岁年龄组Sp阳性率分别为15.68%、21.33%、25.97%(χ^(2)=10.42,P=0.005),Hi阳性率分别为13.31%、24.53%、18.51%(χ^(2)=14.61,P=0.001)。0、1-2、≥3剂次肺炎球菌结合疫苗(PCV)免疫史者Sp阳性率分别为27.26%、29.93%、23.96%(χ^(2)=1.46,P=0.481),0、1-2、≥3剂次Hib疫苗免疫史者Hi阳性率分别为22.80%、27.52%、29.49%(χ^(2)=5.39,P=0.068)。结论 0-16岁AURI就诊儿童Sp和Hi携带率高;需加强适龄儿童PCV和Hib疫苗接种,进一步开展Sp和Hi血清型检测。
Objective To explore carriage of Streptococcus pneumonia(Sp)and Haemophilus influenza(Hi)in children with acute upper respiratory infection(AURI).Methods We obtained throat swab samples from children 0-16 years of age with AURI from March 2021 to June 2022 who were seen in Kunming Children's Hospital.We tested for Sp and Hi nucleic acid using real-time fluorescence quantitative polymerase chain reaction to determine positivity(carriage).Results Among the 1021 subjects,carriage rates of Sp and Hi were 20.86%and 19.00%,respectively,with 6.86%positive for both Sp and Hi.Among 0-2-,3-5-,and 6-16-year-old subjects,carriage rates of Sp were 15.68%,21.33%,and 25.97%(χ^(2)=10.42,P=0.005)and carriage rates of Hi were 13.31%,24.53%,and 18.51%(χ^(2)=14.61,P=0.001).Sp carriage rates among subjects who had received 0,1-2,and≥3 doses of pneumococcal conjugate vaccine(PCV)were 27.26%,29.93%,and 23.96%(χ^(2)=1.46,P=0.481).Hi carriage rates among subjects who had received 0,1-2,and≥3 doses of Haemophilus influenzae type b(Hib)vaccine were 22.80%,27.52%,and 29.49%(χ^(2)=5.39,P=0.068).Conclusions The carriage levels of Sp and Hi were high among children 0-16 years of age with AURI.It is necessary to strengthen PCV and Hib vaccination for age-eligible children and to test for Sp and Hi serotypes.
