摘要
目的 分析应用彩色多普勒超声检查大脑中动脉与脐动脉阻力指标对胎儿缺氧的诊断价值。方法 选取40例产前检查提示胎儿缺氧的产妇作为观察组,另选取40例产前检查提示胎儿正常的产妇作为对照组,两组均实施彩色多普勒超声检查。比较两组胎儿大脑中动脉与脐动脉阻力指标[阻力指数(RI)、搏动指数(PI)以及收缩期峰值流速和舒张期流速比值(S/D)],分析胎儿大脑中动脉与脐动脉阻力指标对胎儿缺氧的诊断效能。结果 观察组胎儿大脑中动脉RI(0.60±0.13)、PI(1.51±0.22)以及S/D(3.88±0.36)均显著低于对照组的(0.79±0.12)、(1.80±0.09)、(4.25±0.34),脐动脉RI(0.81±0.13)、PI(1.76±0.14)以及S/D(3.39±0.37)均显著高于对照组的(0.62±0.09)、(1.09±0.28)、(2.38±0.36),差异具有统计学意义(P<0.01)。将大脑中动脉与脐动脉RI、PI以及S/D作为检验变量,将胎儿缺氧情况作为状态变量,绘制受试者工作曲线(ROC曲线),彩色多普勒超声检测大脑中动脉与脐动脉RI、PI以及S/D单独诊断和联合诊断胎儿缺氧的曲线下面积(AUC)均>0.800,可发挥诊断价值。结论 胎儿缺氧诊断中应用彩色多普勒超声检查大脑中动脉与脐动脉阻力指标可发挥显著诊断价值,继而可为后续采取治疗奠定基础,建议广泛应用于临床中。
Objective To analyze the value of color Doppler ultrasonography in the diagnosis of fetal hypoxia by detecting the resistance indexes of middle cerebral artery and umbilical artery.Methods 40 maternal women whose prenatal examination suggested fetal hypoxia were selected as the observation group,and another 40 maternal women whose prenatal examination suggested fetal normality were selected as the control group,and color Doppler ultrasonography was performed in both groups.The resistance indexes of the middle cerebral artery and umbilical artery[resistance index(RI),pulsatility index(PI),and the ratio of peak systolic velocity to end diastolic velocity(S/D)]between the two groups were compared,and the diagnostic efficiency of the resistance indexes of the middle cerebral artery and the umbilical artery in the diagnosis of fetal hypoxia was analyzed.Results The RI,PI and S/D of middle cerebral artery were(0.60±0.13),(1.51±0.22),and(3.88±0.36)in the observation group,which were significantly lower than those of(0.79±0.12),(1.80±0.09)and(4.25±0.34)in the control group;the RI,PI and S/D of umbilical artery were(0.81±0.13),(1.76±0.14),and(3.39±0.37)in the observation group,which were significantly higher than those of(0.62±0.09),(1.09±0.28),and(2.38±0.36)in the control group;the differences were statistically significant(P<0.01).The receiver operating characteristic(ROC)curve was plotted by using RI,PI and S/D of middle cerebral artery and umbilical artery as test variables and fetal hypoxia as the state variable.Area under the curve(AUC)of RI,PI and S/D of middle cerebral artery and umbilical artery detected by color Doppler ultrasound in the diagnosis of fetal hypoxia alone and in combination were all>than 0.800,which could play a diagnostic value.Conclusion The application of color Doppler ultrasonography in the diagnosis of fetal hypoxia can play a significant diagnostic value in detecting the resistance indexes of middle cerebral artery and umbilical artery,which can lay a foundation for subsequent treatment,and it is recommended to be widely used in clinical practice.
