摘要
目的探讨慢性阻塞性肺疾病急性加重期(AECOPD)患者血清微小RNA-181a(miR-181a)、Smad同源物7(Smad7)mRNA表达变化及其与病情严重程度和气道重塑的关系。方法选择AECOPD患者121例(AECOPD组),病情严重程度:轻度52例、中度46例、重度23例,同期另选慢性阻塞性肺疾病稳定期(SCOPD)患者45例(SCOPD组)、体检健康的志愿者42例(对照组)。采集所有研究对象空腹外周静脉血,离心留取血清,采用RT-qPCR法检测血清miR-181a、Smad7 mRNA表达;采用16排螺旋CT行胸部高分辨率扫描,测量气道腔内径、气道腔外径(D)、气道壁面积(WA)、气道总横截面积,计算气道壁厚度(T)与D比值(T/D)、WA占气道总横截面积百分比(WA%)。比较三组血清miR-181a、Smad7 mRNA表达和T/D、WA%,以及不同病情严重程度AECOPD患者血清miR-181a、Smad7 mRNA表达和T/D、WA%。分析AECOPD患者血清miR-181a表达与Smad7 mRNA表达的关系以及血清miR-181a、Smad7 mRNA表达与病情严重程度和气道重塑的关系。结果AECOPD组血清miR-181a、Smad7 mRNA表达均低于SCOPD组和对照组,T/D、WA%均高于SCOPD组和对照组(P均<0.05);SCOPD组血清miR-181a、Smad7 mRNA表达均低于对照组,T/D、WA%均高于对照组(P均<0.05)。随着病情严重程度增加,AECOPD患者血清miR-181a、Smad7 mRNA表达逐渐降低,T/D、WA%逐渐升高(P均<0.05)。Pearson相关分析显示,AECOPD患者血清miR-181a表达与Smad7 mRNA表达呈正相关关系(P<0.01)。Spearman相关分析显示,AECOPD患者血清miR-181a、Smad7 mRNA表达与病情严重程度和T/D、WA%均呈负相关关系(P均<0.01)。结论AECOPD患者血清miR-181a、Smad7 mRNA表达降低,二者表达变化与病情严重程度和气道重塑密切相关。
Objective To investigate the expression changes of serum microRNA-181a(miR-181a)and Smad homolog 7(Smad7)expression in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and their relationships with disease severity and airway remodeling.Methods Totally 121 patients with AECOPD were selected(AECOPD group),with severity of disease:52 mild,46 moderate and 23 severe cases,while 45 patients with stable chronic obstructive pulmonary disease(SCOPD)(SCOPD group)and 42 healthy volunteers(control group)were selected during the same period.Fasting peripheral venous blood was collected from all study subjects,serum was centri⁃fuged,and serum miR-181a and Smad7 mRNA expression levels were detected by RT-qPCR;16-row spiral CT was used to perform chest high-resolution scanning,and airway lumen inner diameter,airway lumen outer diameter(D),airway lumen area(WA),and total airway cross-sectional area were measured,and airway wall thickness(T)to D ratio(T/D)and airway wall area as a percentage of total bronchial cross-sectional area(WA%)were calculated.We compared serum miR-181a and Smad7 mRNA expression as well as T/D,WA%in three groups and serum miR-181a and Smad7 mRNA expression as well as T/D,WA%in AECOPD patients with different severity of disease.We analyzed the relationship between serum miR-181a expression and Smad7 mRNA expression and the relationships between serum miR-181a and Smad7 mRNA expression and disease severity and airway remodeling in AECOPD patients.Results Serum miR-181a and Smad7 mRNA expression levels in the AECOPD group were significantly lower than those in the SCOPD and control groups,and T/D and WA%were significantly higher than those in the SCOPD and control groups(all P<0.05);serum miR-181a and Smad7 mRNA expression in the SCOPD group were significantly lower than those in the control group,and T/D and WA%were significantly higher than those in the control group(all P<0.05).As the degree of disease grew worse,serum miR-181a and Smad7 mRNA expression levels gradually decreased and T/D and WA%gradually increased in patients with AECOPD(all P<0.05).Pearson correlation analysis showed that serum miR-181a expression was positively correlated with Smad7 mRNA expression in patients with AECOPD(P<0.01).Spearman correlation analysis showed that serum miR-181a and Smad7 mRNA expression levels in AECOPD patients were negatively correlated with disease severity and WA%and T/D(all P<0.01).Conclusion Serum miR-181a and Smad7 mRNA expression was significantly reduced in patients with AECOPD,and their reduced expression was closely associated with disease severity and airway remodeling.
