摘要
慢性心力衰竭(慢性心衰)是各种病因引起的心血管疾病的严重或终末阶段,其不但表现为心脏泵功能的减退,还合并肾脏、肝脏、肺脏等其他脏器功能的损害,其中肝脏的继发性损伤是最为常见的一种。中医认为慢性心衰发生的原因错综复杂,多由各种心脏自身病变及他脏疾病及心迁延日久,致使心气亏虚无力运行血脉,形成了在本虚的基础上产生血瘀、痰浊、水饮等标实证,其病位在心,涉及肝脾肾诸脏,治疗常兼顾五脏六腑、气血津液、阴阳五行等方面。若肝气郁滞,疏泄无力,势必加重心气推动之力,久之则可致使心气亏虚,从而在临床实践中逐渐形成“心肝相关”理论,从“心肝相关”理论出发,可以加深对慢性心衰病因病机的理解及提高确切的治疗效果。因此文章以“心肝相关”理论为出发点,论述了心与肝在阴阳、五行、经络、气血、情志等生理方面相互关联,病理方面相互影响,探讨了治心勿忘治肝,心肝同治的慢性心衰诊疗方法,以期为慢性心衰的治疗提供新的思路。
Chronic heart failure is a serious or final stage of cardiovascular diseases caused by various causes,which is not only characterized by the decline of cardiac pump function,but also complicated with the damage of kidney,liver,lung and other organs,among which the secondary damage of liver is one of the more common ones.Traditional Chinese medicine believes that the causes of chronic heart failure are complex,and most of them are caused by various heart diseases,other Zang-Fu organs diseases and deficiency of heart-Qi and inability to run blood vessels because of long-term heart disease,forming blood stasis,phlegm turbidity and water retention on the basis of deficiency of origin,and its disease is located in the heart and involves the liver,spleen and kidney.The treatment often takes into account five-Zang organs,Qi,blood and body fluid,Yin and Yang and five elements,etc.Stagnation of liver-Qi and weakness of dredging and discharging will aggravate the driving force of heart-Qi,and it will lead to deficiency of heart-Qi,thus gradually forming the theory of“heart and liver being related”in clinical practice.Starting from the theory of“heart and liver being related”,it can deepen the understanding of chronic heart failure and the understanding of etiology and pathogenesis and improve the exact therapeutic effect.Therefore,starting from the theory of“heart and liver being related”,it is discusses that mutual correlation between heart and liver in Yin and Yang,five elements,meridians,Qi and blood,emotions and other physiological aspects and the mutual influence in pathology.This paper discussed the diagnosis and treatment method of chronic heart failure with simultaneous treatment of heart and liver,so as to provide a new idea for diagnosis and treatment of chronic heart failure.
