摘要
目的探讨脑循环经颅磁刺激联合醒脑开窍针刺法治疗脑卒中后抑郁失眠的效果。方法选取2020年11月至2021年11月湖北省第三人民医院康复科收治的80例脑卒中后抑郁失眠患者作为研究对象,采用随机数字表法将其分为对照组(醒脑开窍针刺法,40例)与观察组(脑循环经颅磁刺激联合醒脑开窍针刺法,40例),比较两组患者治疗前后的汉密尔顿抑郁量表(HAMD)评分、匹兹堡睡眠量表(PSQI)评分、日常生活能力评估量表(ADL)评分、简易智力状态检查量表(MMSE)评分、临床治疗有效率。结果治疗前,两组患者HAMD、PSQI评分比较,差异无统计学意义(P>0.05);治疗后,两组患者HAMD、PSQI评分均低于本组治疗前,且观察组HAMD、PSQI评分低于均对照组,差异有统计学意义(P<0.05)。观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗前,两组患者ADL、MMSE评分比较,差异无统计学意义(P>0.05);治疗后,两组患者ADL、MMSE评分均高于本组治疗前,且观察组ADL、MMSE评分高于对照组,差异有统计学意义(P<0.05)。结论在脑卒中抑郁失眠治疗中采取脑循环经颅磁刺激联合醒脑开窍针刺法,临床疗效显著,能够提高临床治疗有效性,消除抑郁心理情绪,改善睡眠质量,值得推荐。
Objective To explore the effect of transcranial magnetic stimulation of cerebral circulation combined with Xingnao Kaiqiao acupuncture therapy on post-stroke depression and insomnia.Methods A total of 80 patients with post-stroke depression and insomnia admitted to the Department of Rehabilitation,the Third People's Hospital of Hubei Province from November 2020 to November 2021 were selected as the research subjects.They were divided into control group(40 cases,using the Xingnao Kaiqiao acupuncture therapy)and observation group(40 cases,using the combination of transcranial magnetic stimulation of the cerebral circulation and Xingnao Kaiqiao acupuncture therapy)according to random number table method,the Hamilton depression scale(HAMD)score,Pittsburgh sleep scale(PSQI)score,the activity of daily living(ADL)and mini-mentalstate examination(MMSE)score before and after treatment and clinical treatment efficiency were compared between the two groups.Results Before treatment,there were no statistically significant differences in HAMD and PSQI score between the two groups(P>0.05).After treatment,the HAMD and PSQI score of the two groups were lower than those before treatment,the HAMD and PSQI score of the observation group were lower than those of the control group,with statistically significant differences(P<0.05).The total effective rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).Before treatment,there were no statistically significant differences in ADL and MMSE score between the two groups(P>0.05).After treatment,ADL and MMSE score of two groups were lower than those before treatment,and the ADL score and MMSE score of the observation group were lower than those of the control group,with statistically significant differences(P<0.05).Conclusion The combination of transcranial magnetic stimulation of cerebral circulation and Xingnao Kaiqiao acupuncture therapy in the treatment of stroke,depression,and insomnia has significant clinical efficacy,can improve the effectiveness of clinical treatment,eliminate depressive psychological emotions,and improve sleep quality,and is worth recommending.
