摘要
目的 探讨血清白细胞介素6(IL-6)、白介素8(IL-8)和可溶性血管细胞黏附分子-1(sVCAM-1)联合检测在早期诊断儿童脓毒血症中的应用价值。方法 选择2020年2月-2022年1月新疆医科大学第一附属医院PICU收治的59例儿童脓毒血症患者纳入脓毒血症组,同时选择同期入院的58例普通感染儿童患者纳入局部感染组,以及60例同期入院体检儿童作为健康对照组。三组纳入研究儿童入院6 h内采集血液样本,对比分析各组血清IL-6、IL-8和sVCAM-1含量,并通过受试者工作特征(ROC)曲线,以评价3项生物标志物单独和联合应用在儿童脓毒血症早期诊断中的价值。结果 与健康对照组相比,局部感染组及脓毒血症组患儿血清IL-6、IL-8和sVCAM-1水平均显著升高,差异具有统计学意义(P<0.01);与局部感染组相比,脓毒血症组患儿血清IL-6、IL-8和sVCAM-1水平进一步升高,差异具有统计学意义(P<0.01)。ROC曲线分析结果显示,血清IL-6、IL-8、sVCAM-1均对儿童脓毒血症具有诊断价值(均P<0.0001)。3项生物标志物联合检测,AUC、灵敏度、特异度、阳性预测值和阴性预测值分别升至0.94、94.74%、90.00%、91.53%和93.75%,均高于上述单一指标检测。结论 血清IL-6、IL-8和sVCAM-1三者联合检测对儿童脓毒血症早期诊断具有重要的应用价值。
Objective To investigate the value of combined detection of serum interleukin-6(IL-6),interleukin-8(IL-8),and the soluble vascular cell adhesion molecule-1(sVCAM-1)in the early diagnosis of Sepsis in children.Methods During the February 2020 to January 2022,a total of 59 children suffered from Sepsis were admitted to PICU of our hospital,and designed as the Sepsis group,58 children with common infection were designed as the common infection group,and 60 healthy children were included into control group.Blood samples were collected from the children in the three groups within 6 hours after admission,and the levels of IL-6,IL-8,as well as sVCAM-1 in serum were compared and analyzed.The receiver operating characteristic curve(ROC)was used to evaluate the value of single and combined detection of the three biomarkers in the early diagnosis of Sepsis in children.Results Compared with control group,IL-6,IL-8,as well as sVCAM-1 in the serum of the common infection group and sepsis group were significantly increased(P<0.01).Compared with the common infection group,the levels of IL-6,IL-8,as well as sVCAM-1 in the serum of the sepsis group were further increased,with statistically significant(P<0.01).Moreover,the analysis of ROC curve showed that the serum of IL-6,IL-8,and sVCAM-1 had diagnostic value for sepsis in children(all P<0.01).The AUC,sensitivity,specificity,positive predictive value and negative predictive value of combined detection of three biomarkers increased to 0.94,94.74%,90.00%,91.53%and 93.75%,respectively,which were higher than those of single biomarker detection.Conclusion The combined detection of three biomarkers in the serum,including IL-6,IL-8,and sVCAM-1,has important application value in the early diagnosis of sepsis in children.
