摘要
目的探讨环磷酰胺联合光动力疗法(PDT)治疗尖锐湿疣(CA)的临床效果。方法收集2017年5月—2020年5月本院就诊的120例CA患者临床资料,按随机数字表法分为A、B、C 3组。A组实施PDT联合25 mg/d环磷酰胺方案,B组实施PDT联合50 mg/d环磷酰胺方案,C组实施PDT联合75 mg/d环磷酰胺方案。术后进行为期3个月的随访,比较3组患者治疗前及治疗3个疗程后的细胞因子指标[白细胞介素-4(IL-4)、IL-10、IL-2和干扰素-γ(IFN-γ)]、免疫功能指标(CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+))、不良反应以及复发率发生率差异。结果与治疗前比较,3组治疗后IL-2、IFN-γ、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均明显升高(均P<0.05),与A组比较,B组和C组升高较为明显,C组升高更为显著(均P<0.05);3组治疗后IL-4、IL-10均明显降低,且C组降低更为显著(均P<0.05)。3组治疗期间不良反应发生率比较差异均无统计学意义(均P>0.05)。3组复发率比较C组<B组<A组[2.50%(1/40)<7.50%(3/40)<20.00%(8/40),均P<0.05]。结论在CA患者中实施PDT联合75 mg/d环磷酰胺方案,能有效诱导IL-2和IFN-γ的表达、抑制IL-4和IL-10的表达,改善患者免疫功能指标,降低复发率,不会明显增加用药不良反应。
Objective In order to explore the clinical effects of cyclophosphamide combined with photodynamic therapy(PDT)on condyloma acuminatum(CA).Methods The clinical data of 120 CA patients treated in our hospital from May 2017 to May 2020 were collected and divided into group A,B and C using a random number table method.Group A implemented the PDT combined 25 mg/d cyclophosphamide regimen,group B implemented the PDT combined 50 mg/d cyclophosphamide regimen,and group C implemented the PDT combined 75 mg/d cyclophosphamide regimen.A three-month follow-up was carried out after the operation to compare the cytokine indicators[interleukin-4(IL-4),IL-10,IL-2 and interferon-γ(IFN-γ)],immune function indexes(CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)),recurrence rate and adverse reaction rate during follow-up.Results Compared with before treatment,IL-2,IFN-γ,CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in the three groups were significantly increased after treatment(all P<0.05).Compared with group A,the increasing in group B and group C were a little more significant,and the increasing in group C was much more significant(all P<0.05).After treatment,IL-4 and IL-10 were significantly decreased in the three groups,and the decreasing in group C was more significant(all P<0.05).There were no significant differences in the incidence of adverse reactions among three groups during treatment(all P>0.05).The recurrence rate of group C was lower than that of group B,and the recurrence rate of group B was lower than that of group A[2.50%(1/40)<7.50%(3/40)<20.00%(8/40),all P<0.05].Conclusion The implementation of PDT combined with 75 mg/d cyclophosphamide regimen in CA patients can effectively induce the expression of IL-2 and IFN-γ,inhibits the expression of IL-4 and IL-10,improve the patient's immune function indicators,and reduce the recurrence rate.No significant increase in adverse drug reactions.
