摘要
目的:探究减重代谢术后患者维生素水平变化与脱发的相关性,并筛选出与脱发相关的独立危险因素。方法:收集2021年3月~9月入住南京大学医学院附属鼓楼医院普外科行减重代谢手术的肥胖患者67例,依据术后3个月门诊随访时的皮肤镜影像学检查诊断分为未脱发组(24例)和脱发组(43例)。比较两组患者术后1月内及3月内维生素水平的变化量,并用Logistic回归分析筛选出影响患者术后脱发的独立危险因素。结果:患者脱发发生率为64.18%(43/67),开始脱发时间为术后(2.88±0.87)个月。脱发组患者术后1月内叶酸和25-OH VD水平的变化量明显低于未脱发组(P<0.05),术后3月内VB_(12)水平的变化量明显低于未脱发组(P<0.05)。多因素Logistic回归分析显示,术后1个月时,叶酸、25-OH VD的变化量是患者发生脱发的独立危险因素(P<0.05);术后3个月时,VB_(12)的变化量是患者发生脱发的独立危险因素(P<0.05)。结论:减重代谢术后1月内低叶酸、25-OH VD水平及术后3月内低VB_(12)水平是术后脱发发生的独立危险因素,推荐术后3月内针对性补充叶酸、维生素D3和VB_(12),并定期进行维生素水平监测。
Objective:To investigate the correlation between postoperative variations in vitamin levels and hair loss in patients post bariatric metabolic surgery,and screen for independent risk factors associated with hair loss.Methods:A retrospective cohort study was conducted to collect 67 obese patients who were admitted to the Department of General Surgery of Nanjing Drum Tower Hospital,the Affiliated Hospital of Nanjing University Medical School for weight loss metabolic surgery from March 2021 to September 2021,and the patients were divided into the group with no or mild hair loss(24 patients)and the group with moderate to severe hair loss(43 patients)based on the diagnosis of dermoscopic imaging at the 3-month postoperative outpatient follow-up.The amount of changes in vitamin levels between the two groups within 1 month and 3 months after surgery were compared,and logistic regression analysis was used to screen out the independent influencing factors affecting patients'postoperative hair loss.Results:A total of 43 patients with varying degrees of moderate to severe alopecia were observed in 67 patients,with an incidence of 64.18%.The time of initiation of alopecia was(2.88±0.87)months after surgery.The amount of change in folic acid and 25-OH VD levels within 1 month postoperatively was significantly lower in the alopecia group than in the non-alopecia group(P<0.05),and the amount of change in VB_(12) levels within 3 months postoperatively was significantly lower than in the non-alopecia group(P<0.05).Multi-factor logistic regression analysis showed that the amount of change in folic acid and 25-OH VD was an independent protective factor for the development of alopecia in patients at 1 month postoperation(P<0.05).The amount of change in VB_(12) was an independent protective factor for the occurrence of alopecia in patients at 3 months postoperation(P<0.05).Conclusion:Low folate and 25-OH VD levels in the first month after bariatric metabolic surgery and low VB_(12) levels in the third month after surgery are independent influencing factors for postoperative alopecia.Targeted supplementation of folate,VD3 and VB_(12) within three months after surgery and regular monitoring of vitamin levels are recommended for patients post bariatric metabolic surgery.
