摘要
目的探讨钬激光前列腺剜除术治疗前列腺增生在基层医院中的安全性和临床价值。方法纳入2021年1月至2022年2月收治的前列腺增生患者70例,随机数字表法分为观察组和对照组,每组35例。对照组采用等离子前列腺剜除术,观察组采用钬激光前列腺剜除术。收集患者围手术期各项指标:术中出血量、剜除组织时间、组织切除量、术后住院时间、术中及术后并发症发生情况,比较2组术前及术后1个月、6个月时国际前列腺症状评分(IPSS)、最大尿流率(Qmax)、生活质量评分(QOL)情况。结果观察组术中出血量、剜除组织时间、术后住院时间均明显低于对照组,组织切除量明显高于对照组(P<0.05);2组患者术后1个月、6个月的Qmax明显高于术前,且观察组高于对照组(P<0.05),而IPSS评分、QOL评分明显低于术前,且观察组低于对照组(P<0.05);2组患者并发生发生率差异无统计学意义(P>0.05)。结论钬激光前列腺剜除术治疗前列腺增生的疗效显著、住院时间短、安全性高,在基层医院更具有应用价值。
Objective To investigate the safety and clinical value of the holmium laser enucleation of the prostate(HoLEP)in the treatment of benign prostatic hyperplasia in grassroots hospitals.Methods A total of 70 patients with benign prostatic hyperplasia admitted to the hospital from January 2021 to February 2022 were included as the study objects.They were randomly divided into the observation group and the control group with 35 patients in each group.Patients in the control group were treated with plasma enucleation of the prostate,and those in the observation group were treated with HoLEP.The perioperative indicators of patients were collected,including the intraoperative blood loss,enucleation time,tissue resection volume,postoperative length of stay,intraoperative and postoperative complications.The international prostate symptom score(IPSS),maximum urinary flow rate(Qmax)and quality of life score(QOL)were compared between the two groups before operation,and 1 month and 6 months after operation.Results Patients in the observation group presented significantly less intraoperative bleeding,shorter time of tissue enucleation and length of stay,and larger tissue resection volume than those in the control group(P<0.05).The Qmax at 1 and 6 months postoperatively in both groups was significantly higher than that before operation,which was significantly higher in the observation group than that in the control group(P<0.05).The IPSS score and QOL score at 1 and 6 months postoperatively in both groups were significantly lower than those before operation,which were significantly lower in the observation group than in the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion HoLEP is effective,and safe with a short length of stay,which has great value in the treatment of benign prostatic hyperplasia in grassroots hospitals.
