摘要
目的探讨胸腔积液中细胞免疫因子表达水平对结核性胸膜炎的诊断价值。方法选取本医共体因胸腔积液就诊的患者90例为研究对象,根据诊断结果将其分为结核性胸腔积液组(n=50)、类肺炎性胸腔积液组(n=20)、漏出性胸腔积液组(n=20)。检测并比较三组患者胸腔积液中白细胞介素27(IL-27)、γ-干扰素(IFN-γ)、白细胞介素10(IL-10)表达水平,评估IL-27、IFN-γ、IL-10对结核性胸膜炎的诊断价值。结果结核性胸腔积液组、类肺炎性胸腔积液组和漏出性胸腔积液组患者的IL-27、IFN-γ、IL-10表达水平比较,差异均有统计学意义(H分别=45.50、26.65、17.53,P均<0.05),其中结核性胸腔积液组患者IL-27水平明显高于漏出性胸腔积液组患者(P<0.05),IFN-γ、IL-10水平明显高于漏出性胸腔积液组和类肺炎性胸腔积液组患者(P均<0.05)。ROC曲线分析显示,患者胸腔积液中IL-27、IFN-γ、IL-10及联合检测对结核性胸膜炎均具有较高的诊断价值,其曲线下面积(AUC)分别为0.77、0.80、0.76、0.87。结论结核性胸膜炎患者胸腔积液中IL-27、IFN-γ、IL-10表达水平较高,其单一检测及联合检测对结核性胸膜炎均具有一定的诊断价值,且联合检测的诊断价值最高。
Objective To investigate the value of cellular immune factor expression in pleural effusion in the diagnosis of tuberculous pleurisy.Methods A total of 90 patients with pleural effusion were selected as research objects and divided into tuberculous pleural effusion group(50 cases),pneumonic-like pleural effusion group(20 cases)and leaky pleural effusion group(20 cases)according to the diagnosis results.The expression levels of interleukin-27(IL-27),interferon-γ(IFN-γ)and interleukin-10(IL-10)in pleural effusion among the three groups were detected and compared,and the diagnostic values of IL-27,IFN-γand IL-10 for tuberculous pleurisy were evaluated.Results The expression levels of IL-27,IFN-γand IL-10 among tuberculous pleural effusion group,pneumonoid pleural effusion group and leaky pleural effusion group were significantly different(H=45.50,26.65,17.53,P<0.05).The IL-27 level in tuberculous pleural effusion group was higher than that in leaky pleural effusion group,and the IFN-γand IL-10 levels were higher than those in pneumonoid pleural effusion group and leaky pleural effusion group(P<0.05).The ROC curve analysis showed that the expression levels of IL-27,IFN-γ,and IL-10 in pleural effusion of patients and their combined detection had high diagnostic value for tuberculous pleurisy,and the area under the curve(AUC)was 0.77,0.80,0.76,and 0.87,respectively.Conclusion The expression levels of IL-27,IFN-γand IL-10 in pleural effusion of tuberculous pleurisy patients are high.The single detection and combined detection had certain diagnostic value for tuberculous pleurisy,and combined detection had the highest diagnostic value.
