硬膜外镇痛分娩方式对初产妇妊娠结局的影响

摘要目的探讨硬膜外镇痛分娩方式对初产妇妊娠结局的影响。方法选择2021年1月—2022年12月在丹阳市妇幼保健院分娩的初产妇200例为研究对象,根据分娩镇痛方式分为未进行分娩镇痛的对照组,硬膜外镇痛分娩方式分研究组,每组100例。对比两组初产妇分娩方式、各产程时间、产后出血量、疼痛程度以及新生儿Apgar评分。结果与对照组自然分娩率(66.00%)、剖宫产率(15.00%)相比,研究组初产妇自然分娩率(81.00%)更高,剖宫产率(5.00%)更低,差异有统计学意义(χ^(2)=5.776、5.556,P<0.05)。与对照组相比,研究组初产妇第一产程、第二产程时间(251.46±32.63)、(50.68±7.36)min均更短,差异有统计学意义(P<0.05)。与对照组相比,研究组初产妇疼痛评分(1.89±0.25)分更低,差异有统计学意义(P<0.05)。结论硬膜外镇痛分娩方式可显著缩短初产妇产程时间,提高其自然分娩率,减轻初产妇疼痛程度,改善其妊娠结局。 Objective To investigate the effect of epidural analgesia delivery modality on pregnancy outcome in primi⁃gravid women.Methods A total of 200 primipara who gave birth in Danyang Maternal and Child Health Hospital from January 2021 to December 2022 were selected as the study objects.According to the methods of labor analgesia,they were divided into control group without labor analgesia and study group with epidural analgesia,with 100 cases in each group.Methods of delivery,duration of each labor stage,postpartum bleeding,pain degree and neonatal Apgar score were compared in the two groups.Results Compared with the natural delivery rate(66.00%)and cesarean section rate(15.00%)in the control group,the natural delivery rate(81.00%)was higher and the cesarean section rate(5.00%)was lower in the research group,and the difference was statistically significant(χ^(2)=5.776,5.556,P<0.05).Compared with the control group,the first and second stages of labor in the study group were shorter(251.46±32.63)min and(50.68±7.36)min,and the difference was statistically significant(P<0.05).Compared with the control group,the pain score of primipara in the study group was lower(1.89±0.25)points,and the difference was statistically significant(P<0.05).Conclusion Epidural analgesic delivery can significantly shorten the labor time of primipara,increase the rate of natu⁃ral delivery,reduce the degree of pain of primipara,and improve the pregnancy outcome.
