摘要
目的分析裸花紫珠胶囊结合生长抑素治疗老年肝硬化上消化道出血患者的临床效果与安全性。方法纳入102例老年肝硬化上消化道出血患者,将其以随机数字表法分为治疗组(51例)与对照组(51例),对照组患者接受生长抑素治疗,治疗组患者接受生长抑素结合裸花紫珠胶囊治疗。比较两组疗效、治疗前后患者中医证候总积分变化、肝功能指标变化与凝血指标变化、不良反应。结果治疗组患者治疗总有效率(96.08%)比对照组(82.35%)更高(P<0.05)。治疗前,两组患者中医证候总积分、总胆红素(TBil)及谷草转氨酶(AST)、谷丙转氨酶(ALT)、凝血酶原时间(PT)及部分凝血活酶时间(APTT)、纤维蛋白原(FIB)比较,差异无统计学意义(P>0.05);治疗后两组患者中医证候总积分、TBil、AST、ALT、PT、APTT、FIB均下降,治疗组低于对照组(P<0.05);治疗组与对照组患者不良反应率均较低,差异无统计学意义(P>0.05)。结论裸花紫珠胶囊结合生长抑素治疗老年肝硬化上消化道出血患者的临床效果良好,患者治疗后症状改善,肝功能恢复较好,不良反应少,治疗可靠、安全。
Objective To analyze the clinical effect and safety of Luohua Zizhu capsule combined with somatostatin in the treatment of upper gastrointestinal bleeding in elderly patients with cirrhosis.Methods A total of 102 elderly cirrhotic patients with upper digestive tract hemorrhage were included in this study,and they were divided into treatment group(51 cases)and control group(51 cases)by random number table method.Patients in the control group were treated with somatostatin,and patients in the treatment group were treated with somatostatin combined with Luohua Zizhu capsule.Curative effect,changes in the total score of TCM symptoms,changes in liver function index and coagulation index of patients before and after treatment,adverse reactions were compares between the two groups.Results The total effective rate of treatment group(96.08%)was higher than that of control group(82.35%)(P<0.05).There were no differences in TCM syndrome score,total bilirubin(TBil),aspartate aminotransferase(AST),alanine aminotransferase(ALT),prothrombin time(PT),partial thromboplastin time(APTT),fibrinogen(FIB)between the two groups before treatment(P>0.05).After treatment,the total scores of TCM symptoms,TBil,AST,ALT,PT,APTT and FIB were all decreased,and these indexes in the treatment group were lower than those in the control group(P<0.05).The rate of adverse reactions was lower in treatment group and control group,and the difference was not statistically significant(P>0.05).Conclusion Luohua Zizhu capsule combined with somatostatin has a good clinical effect in the treatment of upper gastrointestinal bleeding in elderly patients with cirrhosis.After treatment,patients’symptoms improved,liver function recovered well,and adverse reactions were less.The treatment is reliable and safe.
